A Study On Myocardial Injury After Aneurysmal Subarachnoid Hemorrhage

Objective:To investigate the impact of related factors to myocardial damage and mortality through observing the changes of electrocardiographic and increased concentrations of serial cardiac markers in patients after aneurysmal subarachnoid hemorrhage (aSAH). Methods: Consecutive patients admitted with aSAH were diagnosed by CTA/DSA and enrolled in the Neurosurgery department of Luzhou Medical Collage between July 2010 and January 2011. Predictor variables reflecting demographic (age, sex, hunt-hess scre, fisher score) were recorded. Serial cardiac troponin I, creatine kinase-MB, myoglobin measurements and ECG were performed on days 1,3, and 7 after enrollment. Dichotomous variables (sex,age,fisher grade group,hunt-hess grade group) were examined with the chi(2) test or the Fisher exact test to determine whether ECG abnormalities and increased concentrations of serial cardiac markers show significant difference between variables groups. The incidence of each polymorphism on the risk of developing electrocardiographic (ECG) abnormalities and increased concentrations of cardiac markers was quantified using multivariable logistic regression. Results:There was no significant difference in the groups(age, sex, Hunt-Hess score, Fisher score) on ECG abnormalities and increased concentrations of MYO. But, there was significant difference in the groups(Hunt-Hess score, Fisher score) on the increased concentrations of TnI and CK-MB. By multivariate logistic regression, age, sex, hunt-hess score, fisher score are not an independent risk factor of ECG abnormalities and increased concentrations of cardiac markers of patients with aneurysmal SAH. Conclusion:ECG changes and increase of cardiac troponinâ… ,creatine kinase-MB,myoglobin were common and irreversible in patients with aneurysmal SAH, but for more time. The key step of the diagnosis and treatment of cardiorespiratory fitness measured by ECG and cardiac troponin I,creatine kinase-MB, myoglobin measurements for patients with high hunt-hess score or fisher score aneurysmal SAH.
