Ojective:This paper studied H pylori infection influence of various factors, Compared the quadruple therapy containing bismuth agent and third-line therapy eradication effection of helicobacter pylori, the cost-effectiveness ratio of them. After the failure of triple radical, We carried through the quadruple rescue therapy to find H. pylori eradication rate, and calculate the cost-benefit ratio.Material and methods:We selected 400 cases of clinic patients with gastrointestinal diseases Since 2007 years, aged 17-80 years old, diagnosis of helicobacter pylori infection was assessed by using 14C-urea breath test, A questionnaire survey including many items related to H pylori infection was conducted, The items included age, gender, dietary habits, parents education level, hygiene, family income etc, single factor x2 inspection and multi-factor unconditional Logistic analysis were used.320 patients with H pylori positive were randomly divided into 4 groups:A1(AAC):esomeprazole,amoxici-llin, clarithromycin sustained-release.A2(ATC):esomeprazole,metronidazole, clarithromycin sustained-rele-ase,B1(ABLF):esomeprazole,colloidal bismuth pectin,levofloxacin,furazolidoneB2(ABTF):esomeprazole,colloidal-bismuth pectin,metronidazole,furazolidone.Follo w-up"C-UBT was performed at least 4 weeks after the end of treatment and at least 2 weeks after taking PPI to assess the treatment response. We observed four groups H.pylori eradication rate,through questionnaire survey observing symptoms, medication side-effects, and the cost-benefit analysis. After the failure of triple radical, We carried through the quadruple rescue therapy to observe two groups H.pylori eradication rate.28 patients with H pylori positive were randomly divided into 2 groups:B1 (ABLF):esomeprazole,colloidal bismuth pectin,levofloxacin,furazolidone.B2(ABTF):esomeprazole,colloidal-bismuth pectin,metronidazole,furazolidone.we compared two rescue therapy eradication rates, symptoms improvement and the cost-benefit ratio.Results:400 people accomplished questionnaires and "C-UBT test, The result of single factor analysis and multi-factor unconditional Logistic analysis showed that the dietary factors for example preferenceof spicy food, eating fresh fruits and vegetables, drinking lactic acid products, water and sanitation conditions for example brushing teeth, toilet quantity, family income, parents cultural level H.pylori infection related (P<0.05), with the related to the influential factors and possibly related factors, we selected more factors unconditional Logistic analysis, it is concluded that Helicobacter pylori infection was relative significantly to age, smoking, alcohol consumption, mother education level, drinking lactic acid products, eating fresh fruits and vegetables, often at home for dinner,brushing teeth, toilet facilities.drinking lactic acid products, often at home for dinner, frequently brushing were protective factors, which could reduce H. py lori infection.320 patients were selected H.pylori eradication therapy, the actual number of cases were 309 cases, they were randomly divided into 4 groups, We draw a conclution that quadruple therapy eradicate rate was higher than third therapy, the difference was statistically significant (P<0.05), ABLF group H. pylori eradication rate was highest, cost-benefit ratio was the minimum,so it could be counted as a kind of ideal first-line treatment. Four groups medication side-effects, ABLF group was slightly high, but the difference was not statistically significant (P>0.05), for symptoms improvement, there were no significant difference (P>0.05).Second-line salvage therapy ABLF eradication rates was obviously higher than ABTF, ABLF cost-benefit was lower than ABTF, for symptoms improvement, there were no significant difference (P>0.05). Conclution:1.Helicobacter pylori infection was may relative to age, parents education level, source of water, eating fresh fruits and vegetables, drink lactic acid products, sanitation facilities, family income.smoking and drinking with Helicobacter pylori relationship remained to be studied.2.Quadruple therapy,including:esomeprazole,colloidal bismuth pectin,levofloxacin, furazolid-one, eradicate rate was higher than third therapy,it cost-benefit ratio was low, which cound be served as Helicobacter pylori first-line therapy.3.After the failure of triple radical,the quadruple rescue therapy after were executed, quadruple therapy of esomeprazole,colloidal bismuth pectin,levofloxacin,furazolidone eradicate rate was higher than quadruple therapy of metronidazole,it cost-benefit ratio was low than quadruple therapy of metronidazole,Which cound be served as preferred solution for second-line salvage therapy.
