Objective Severe brain injury models were established in adult rats by free falling objects; To study the effect of San huang xie xin Tang on the histology and the expressions of nuclear-κB and interleukin- 6 in brain and gastric mucosa of rats wits the acute gastric mucosa lesion after severe traumatic brain injury, and explore San huang xie xin Tang,s effect and mechanism on the acute gastric mucosa lesion in rats with severe brain traumatic injury.Methods 70 wistar rats were randomly divided into six groups. All rats were randomly divided into normal control group (10 rats), model group (30 rats), and Chinese traditional medicine group (30 rats). The two later groups at hour 6.24.72 post-trauma (each group 10 rats).Except normal control group, severe brain injury model were established in other groups by free falling objects; Then, San huang xie xin tang decoction contained 1g/ml drug and saline were administered respectively to Chinese traditional medicine group and normal group and model group by gavage (1 mL/100g every time),once every morning. The neural function of rats injuried were scored after been injured six hours;A series of stomach samples were obtained directly 6.24 and 72 hours respectively after operation in every groups. Normally paraffin sections of brain tissue and gastric mucosa were stained with HE. The areas of lesion and surrounding tissue of brain tissue and gastric mucosa were observed by microscope, and image analysis software was used to total the area of damage. Immunochemistry(IHC) was used to detect the expression of IL-6,NF-ΚB on brain and stomach at different time point groups.Results1. After being made model six hours, the score of Neurologic Impairment in injured groups > 2 and there was no significant difference between groups; Each injured group of the brain has the obvious, widely subarachnoid hemorrhage; in the model and Chinese medicine groups we can see extensive contusions in the left parietal lobe which near the small iron chain, (under a light microscope.) as well as red cell, neuron edema dropsy in different degree, inflammatory cell infiltrate, nerve cell necrosis.2. There are 24 rats(model 6h group2,model 24h group 6,model 72h group 4,chinese 6h medicine group 3,chinese 24h medicine group 7,chinese 72h medicine group 2)total 60,we can see the blood spot in the superficial mucosa, and after flushed with saline, there are some different degrees spot or linear brown hemorrhage in gastric body; In model groups the gastric mucosa appeared defect as well as inflammatory cell infiltrated, formed superficial erosion , conquestion and edama nearby; In the Chinese traditional medicine groups especially in the 72h group the mucosal damage could be lessened and the edema in capillary vessel and interstitial tissue could be alleviated.3. NF-κB and IL-6 were stained in endochylema of brain. Every group of rats expressed,and the number of positive cells achieve peak Compared with the 24 hour; after 72 hours the positive cells slow down. Compared with control group, the expression of IL-6 and NF-κB in the brain tissue of model groups and Chinese traditional medicine 24h group were significantly increased(P<0.01~0.05);Compared with model 72h group,The expression of IL-6 and NF-κB in the brain tissue of Chinese traditional medicine 72h group were obviously decreased(P<0.01~0.05).4. Compared with control group, the expression of IL-6 and NF-κB in the gastric tissue of model 24h and 72h groups were significantly increased(P<0.01~0.05);Compared with model 24h group,The expression of IL-6 in the gastric tissue of Chinese traditional medicine 24h group were obviously decreased(P<0.05); Compared with model 72h group and the same 24h group,The expression of IL-6 and NF-κB in the gastric tissue of Chinese traditional medicine 72h group were obviously decreased too(P<0.01~0.05).5. In brain as well as in stomach the expression of NF-κB and IL-6 is positively correlated.Conclusion1. San Huang Xie Xin Tang can ameliorate the histological changes of gastric mucosa in rats with severe brain traumatic injury. 2. San Huang Xie Xin Tang inhibits the inflammatory reaction by inhibiting the expressions of nuclear-κB and interleukin 6, which may be one of the central mechanisms of its stomach -protective effects.
