Objective To clarify radiofrequency thermocoagulation on vidian and ethmoidal nerves enrichment Regregion to treat allergic rhinitis via nasal endoscope on nasal airway resistance, transport function of nasal cilia and ultrastructure of nasal cilia,provide a theoretical basis for clinical application。Methods random select 36 examples allergic rhinitis patients as experimental group who accepted nasal cavity vidian and ethmoidal nerves radiofrequency thermocoagulation from May 2008 to May 2009。random select 30 examples health adultes as control group。experimental group undertake acoustic rhinometry , nasal airway resistance and saccharin test in preoperative and after follow-up 6 months,control group also do the same thing,the experimental group experimental result in preoperative compared with after follow-up 6 months and the experimental group experimental result in preoperative compared with control group。This study also received double-flank tube,ethmoidal nerve cluster zone radiofrequency ablation surgery in patients with perennial allergic rhinitis mucosa of inferior turbinate surgery before and after 6 months-line transmission electron microscope examination, In addition,in the control group randomly selected Remove the two cases of normal inferior turbinate mucosa-line transmission electron microscope examination。Results saccharin test:the nasal mucociliary transport rate of experimental group were 6.2083±1.87728 mm·min-1 in preoperative and 9.9669±1.73760 mm·min-1 after follow-up 6 months。the nasal mucociliary transport rate of control group was 9.8973±1.58762 mm·min-1。Acoustic rhinometry and nasal airway resistance test:the nasal minimal cross-sectional area of experimental group were 0.7050±0.09142 cm2 in preoperative and 0.9767±0.18025 cm2 after follow-up 6 months,the nasal minimal cross-sectional area of control group was 0.9963±0.18037 cm2 ; the nasal cavity volume from 0-5cm of experimental group were 7.9094±1.01176 cm3 in Preoperative and 11.2167±1.75845 cm3 after follow-up 6 months,the nasal cavity volume from 0-5cm of control group was 11.5880±2.00104 cm3;the nasal inspiration total resistance of experimental group were 0.3743±0.06093 Kpa/L·S in Preoperative and 0.2130±0.04319 Kpa/L·S after follow-up 6 months,the nasal inspiration total resistance of control group was 0.2250±0.05082 Kpa/L·S ; the nasal expiration total resistance of experimental group were 0.3183±0.05907 in preoperative and 0.1811±0.03783 after follow-up 6 months,the nasal expiration total resistance of control group was 0.1885±0.04641 Kpa/L·S。there were significant deviation between the preoperative of experimental group compared with control group and experimental group interclass(P<0.05)。observe nasal mucosa ultramicrostructure at transmission electron microscope in preoperative test:the microvilli and pilus escape from the superficies of membrana mucosa cellula columno epithelialis,the length and duration pilus crisscross,pilus and un-pilus crisscross,endothelial cell diastema enlargement , cellula columnoepithelialis endochylema and chondriosome hydropsia,cellula columnoepithelialis glutinous,basement membrane exposuring and thickening,fraction of mucous membrane fall off,mucous gland and serous gland secretory granule under nasal mucosa underlayer abundance,there are acidophilia and lymphocyte under nasal mucosa underlaye。nasal mucosa ultramicrostructure at transmission electron microscope that the control group and the experimental group after follow-up 6 months display consistency:membrana mucosa cellula columno epithelialis pilus line up in order,directions with one accord,crudeness or fineness uniformity, length and duration with one accord,the pilus were notabscission and lodging , the"9+2"micro-tubular structure of columnar epithelium lampros,chondriosome slender and pykno。Conclusions radiofrequency thermocoagulation on vidian and ethmoidal nerves enrichment Regregion to treat allergic rhinitis via nasal endoscope can raise the nasal mucociliary transport rate,and can improve transmission function of nasal mucosa pilus ,and can obviously reduce nasal airway resistance , and can make the nasal minimal cross-sectional area and the nasal cavity volume from 0-5cm recover。This procedure does not damage the nasal mucosa, and can the already damaged nasal cilia system be improved。...
