Objective Infectious diseases through blood transfusion has been the main problem of blood transfusion safety. The safety of blood transfusion is increasingly widespread concern by the whole society. Through blood tests, to avoid the risk of disease transmission with blood into the clinic is a key measure to ensure safety of blood transfusion. In accordance with relevant laws and regulations, the blood transfusion services must test blood for transfusion transmission of the following five indicators of early resumption of tests:Alanine Aaminotransferase (ALT), Hepatitis B virus surface antigen (HBsAg), Hepatitis C virus antibody (anti-HCV), Human immunodeficiency virus antibody (anti-HIV), Treponema pallidum antibody (anti-TP),and discard the unqualified blood as useless. This paper summarizes the blood test results in the past five years in Shandong Blood Center, analyzes the test results of the positive rate, investigates the main reasons for the blood discarded, study the ALT and HBsAg rapid test results before collecting blood, and analyzes the influence of rapid detection to reducing blood test scrapped in order to minimize blood scrap, guide rapid detection of blood collected before the launching, protecte the health of donor, ensure the safety of clinical transfusion; At the same time, by comparing the ALT reject ratio in different gender, the difference of test result collected before and after holidays, we hope to explore a feasible method that both reducing the blood rejection rate and saving time and cost.Method Reference to Blood Donor Screening Requirements, Basic Standards of Blood and Related Documents, we perform quick testing on ALT and HBsAg to donors before blood collecting, and do early primary screening and re-examination to ALT, HBsAg, anti-HCV, anti-HIV, syphilis after blood collecting. Only about 50% of blood donors were examined for HBsAg by rapid screening in 2007 and 2008, but all blood donors were examined for HBsAg in 2009. All male blood donors were carried out for rapid detection of ALT since 2010.We statistics the number of cases of blood testing failed from 2005 to 2009 on ALT, HBsAg, anti-HCV, anti-HIV, anti-TP, and calculate the failure rate. The cases that blood discarded caused by the various reasons and the positive rates of all indices were compared with pie chart and histogram respectively each year; The failure rate of ALT in gender and before and after holidays were compared with chi-square test; ALT rapid test results and test results of automatic biochemical analyzer on 30 blood samples were compared with rank sum test.Results1.Of all the blood discarded, ALT failure accounted for 75%, HBsAg positive accounted for 20%. Carrying out the fast detection before blood collecting makes the HBsAg positive rate decreased year by year. This is reflected in the fast detection of progressive implementation in 2007 and 2008. In 2008, after implementing the rapid detection of all blood donors, the positive rate decline further.2.There are significant differences on ALT failure rate in different months on each year. Each 1,2,7,8,9,10 months, ALT test failure rate was significantly higher than those in other months. Significant failure rate during the festival ALT was significantly higher than those in the same period, non-holiday.3.On implementation of ALT rapid detection and automatic biochemical analyzer on 30 blood samples, test results were compared. Roche Reflotron Plus ALT dry fast analyzer test results, and OLYMPUS AU640 automatic biochemical analyzer instrument made in Japan results are consistent.4. Comparison of Substandard ALT to Blood donors of different gender, ALT failure rate of blood donors of different gender are significantly different. ALT failure rate of male blood donors were significantly higher than femalesConclusion The ALT and HBsAg were the common Unqualified items in blood testing. Implementing quickly testing for this two items, will Significantly reduce the blood test scrapped. Reflotron Plus ALT dry quick biochemical analyzer is suit for blood collecting vehicle. The reject ratio of ALT in male is significantly higher than that in female, suggesting that that we can only do that to male, if we could not do fast test to all donors, this will save cost in maximum limit. The effective ways to reducing the scrap rate blood test shall be carried out ALT rapid detection before collecting blood, to avoid not meet the requirements of blood donors.
