The Clinical Study Of Diagonosis And Therapy Of Pulmonary Infection After Renal Transplantation

Objective:To evaluate the value of chest CT combined with the culture of sputum and the analysis of blood cell in the early diagonosis of pulmonary infection after renal transplantation,To study the features and diagnosis and treatment of pulmonary infection after renal transplantation.Material and Methods:Fifty two patiens with pulmonary infection after renal transplantation were selected of the objects of this study.Their radiological feartures,laboratory examination and therapis were analyzed retrospectively, and designed control group,we classify them according to the diagonosis of treatment and the detect of etiology,to study the feartures of every etiology on high-resolution CT, prospectively study the value of chest CT combined with the culture of sputum and the analysis of blood cell in the early diagonosis of pulmonary infection after renal transplantation on 37 cases,and observe the clinical effect of combination therapy.Resultes:The infection was caused by germs in 48 cases(68.57% of all patients); fungus 13 cases (18.57%); CMV 6 cases(8.57%); TB 3 cases(4.29%);64.46% of the patients was caused by mixed infection,38 cases were cured,14 were dead,31 cases were checked with chest CT,52 cases were checked with chest X-ray,43 cases with the culture of sputum, the mean time in hospital was 26 days;In the prospectively study on the 37cases, the infection was caused by germs in 33 cases(67.35% of all patients);,fungus 11 cases (22.45%); CMV 3 cases(6.12%); TB 3 cases(4.08%);no etiology 1 case;72.93%(27/37) of the patients was caused by mixed infection,34 cases were cured,3 were dead,,the cure rate was 91.89%,obviously higher than the control group(p<0.05), the mean time in hospital was 21 days,there was statistical significance compared with the control group(p<0.05).Conclutions:The chest CT can find the focus of infection earlier 3-5 days than chest X-ray,it is very important to use chest CT combined with the culture of sputum, the analysis of blood cell for identification in early diagonosis of pulmonary infection after renal transplantation.It is vital for patients to adopt combination therapy.
