Thoracolumbar burst fracture is one of the common spinal injury in the clinical. Its easily leads to unstable spinal segment and leads to spinal cord injury.How to evaluate the stability of thoracolumbar burst fracture and the impact of intraspinal degree to spinal cord, it lacks consensus currently. In this study,from July 2003 to December 2009, collecting 146 patient data of thoracolumbar burst fracture in a hospital. Combining the clinical examination, and X-ray, CT, MRI and so on. Multianalyzing the stability of thoracolumbar burst fracture and the correlation of spinal cord injury, it provides a theoretical basis for clinical treatment..Objective:For the anatomy and classification of thoracolumbar burst fracture, combining the clinical examination, studying X ray, CT and MRI of thoracolumbar burst fracture, to propose the evaluation method of fracture stability, to analyze the correlation of intraspinal and spinal cord injury, to guide the clinical treatment.Methods:Through a review of 146 cases of patients with thoracolumbar burst fracture of the clinical data, based on three-pillar system,combining the classification of thoracolumbar burst fracture, to propose the evaluation indexes by clinical examination, X-ray, CT, MRI performance. The criterion is stability of improved Denis spinal fractures, combining AO fracture classification and stability of McAfee thoracolumbar burst fracture to statistical analysis and draw ROC curve. At the same time,according to different fracture site and degree of spinal bone that CT measured in the correspo-nding plane,to analyze the possibility and correlation of spinal cord injury.Results:Calculating the area of ROC curve,the results of two test methods are> 0.9,area (AO)= 0.936, area (McAfee)= 0.928. Its show that use this scoring method to judge sensitivity and reliability of thoracolumbar burst fracture stability is higher. The average degree of intraspinal bone occupying are T1131%, T1242%, L148%,L255% among different fracture sites. The injury of spinal cord nerve and the degree of intraspinal bone occupying are significant correlation. The correlation with the fracture sites are T12 highest and L2 lowest.Conclusions:To determine the stability of thoracolumbar burst fracture, this method has high accuracy, it is a practical method, and can provide a reliable basis for clinical treatment. The degree of spinal bone occupying among thoracolumbar burst fracture is an important factor in spinal cord injury. The spinal cord injury and its degree related with spinal fracture site and the degree of intraspinal bone occupying.
