Objective: To study the techniques of orthotopic unilateral lung transplantation in rats, and make the orthotopic unilateral lung transplantation model in rats more simple, effective, safe and reliable. Methods: 1. Fourty-six male Sprague-Dawley rats were divided into 23 pairs for donors and recipients randomly. Cuff techniques were introduced in anastomosis of pulmonary artery, pulmonary vein and bronchus in the operation. The graft function was monitored by thoracic X ray, Multi-slice CT scan, arterial blood gas analysis and pathological changes under the light microscope.2. The data of the first 80 consecutive cases of unilateral orthotropic left rat lung transplantation were collected for retrospective study. The learning process of establishing the model was divided into four stages, with 20 cases in each. The donor lungs harvesting time , cuff techniques time, anastomosis time and complications were analyzed in each stage, and modeling techniques were reviewed. Results: 1. In the 23 transplantations, the operation was successfully carried out in 20 cases and failed in 3 cases. The reasons of 3 failed cases were as follows: disrupture of pulmonary vein, disrupture of pulmonary artery and bleeding after operation. Time of donor lung harvesting, cuff techniques and anastomosis was (11.6±2.1) minutes, (13.6±2.4) minutes, (17.7±3.5) minutes respectively. Thoracic X ray, Multi-slice CT scan, arterial blood gas analysis and pathological changes after transplantation manifested excellent graft function.2. With the development of training, the donor lungs harvesting time, cuff techniques time, anastomosis time and complications decreased significantly. The model was established steadily at stageⅢand stageⅣ.Conclusion: With the study of the cuff preparation, techniques of separating pulmonary blood vessels and bronchus, donor lung perfusion pressure, donor lung preservation, tracheal intubation of recipient rats, the choice of recipient rats'incision, pulmonary blood vessels and bronchus anastomosis, perioperative management and complications, a more simple and effective rat model of orthotopic unilateral lung transplantation has been established.
