Objective: To evaluate the long-term results between laparoscopic assisted versus open radical resection for colon cancer.Method: 287 patients with colon cancer underwent radical surgery from September 2000 to December 2008. According to the include criteria, 273 cases left and divided into two groups prospectively and nonrandomly, and received laparoscopic assisted radical surgery (LP, n=147) and open surgery (OP, n=126). Overall rates of recurrence, local recurrence rates, metastasis rates, overall survival and disease-free survival were compared between the two groups.Results: No difference was seen between the two groups on the patients′gender, age, tumor location, TNM stages, gross morphology, histological type, differentiation degree, past history of abdominal surgery and postoperative chemotherapy . (P>0.05). As of May 1, 2009, 19 patients (LP, n=9, OP, n=10) have recurred and 34 patients (LP, n=21, OP, n=13) have metastasis. There is one case in the two groups combined with recurrence and metastasis. 33 patients died from cancer (LP, n=15, OP, n=18). Overall recurrence rates are similar between the two groups (LP 19.7%, OP 17.5%, P=0.632). The 1-year, 2-year, 3-year overall recurrence rates in laparoscopic group are 10.6%, 18.7% and 26.7%, versus open group 7.1%, 15.8% and 20.8%, with no significant difference between two groups (P=0.268). Local recurrence rates (LP 6.1%, OP 7.9%) are also similar between the two groups, with no significant difference between groups in location and time to recur (P=0.643). Metastasis rates (LP 14.3%, OP 10.3%) are similar between the two groups, with no significant difference between the two groups in type and time to metastasis (P=0.168). Overall 3-year survival (LP 86.3%, OP 82.8%) and overall 5-year survival (LP 70.3%, OP 79.9%) between LP and OP groups have no statistical significance (P=0.464), regardless of TNM stages. Disease-free 3-year survival (LP 73.2%, OP 79.3%) and disease-free 5-year survival (LP 62.1%, OP 77.5%) between LP and OP groups have no statistical significance too (P=0.268), regardless of TNM stages.Conclusion: Laparoscopic assisted resection for colon cancer is the same as open surgery.
