Objective:To study the clinical hearing characteristics of auditory neuropathy, including the children with central low-tone sensorineural hearing loss.Method:The experiment was divided into four groups, which were 14cases (27ears) with central low-tone sensorineural hearing loss patients(AN),10 cases (13 ears) with low-tone sensorineural hearing loss,10 cases (20 ears) with high-frequency sensorineural hearing loss, 10 cases (20 ears) with normal hearing.all cases were observed by clinical audiology testing. collection history and ear examination, including acoustic immittance, ABR, evoked otoacoustic emissions (evoked EOAEs) and event-related potentials (ERP) and the peripheral nervous system checks.Results:In auditory neuropathy cases, the hearing loss were mainly base on low-frequency, of moderate-severe hearing loss; Tympanometry were A-type tympanogram; Stapedial acoustic reflex did not elicit. TEOAE examination:in all cases have led to the frequency band, the band regeneration rate>90%. DPOAE examination:there were five ears did not rise to the existence of a frequency which was low frequency. ABR examination:one ear only leads to the extended latency of V wave, then none of ears leads toâ… ,â…¢,â…¤wave, including the case of unilateral auditory neuropathy. The general low-frequency hearing loss group were mainly single-ear disease, hearing loss with low-frequency-based, were mild to moderate hearing loss. Tympanometry were A-type tympanogram, part of the stapedial acoustic reflex leads to (9/13). TEOAE examination:low-frequency components in all cases were of loss or deletion, band regeneration rate<70%, high-frequency components were well-received. ABR examination:only 1 case (2 ears) were normal threshold, then all of ears were higher threshold, but the latency ofâ… ,â…¢,â…¤wave were normal. The high-frequency hearing loss group, hearing loss with high-frequency-based, were moderate and severe hearing loss, were type A tympanogram, stapedial acoustic reflex in low-frequency were extraction,in 4000HZ were not led.TEOAE examination:each band in all cases only a low-frequency band individual led, each band the average regeneration band rate< 40%. DPOAE examination:all ears were only one or two frequencies led. ABR examination:one ear only leads to V wave, then all of ears led toâ… ,â…¢,â…¤wave, the waves were well differentiated, with the normal lantency. Normal hearing control group, all pure-tone hearing thresholds were<25dB, tympanic photo shows A-type, stapedial acoustic reflex were led. TEOAE:all cases were led to the band, band regeneration rate> 90%. ABR examination:threshold of 20 ears were normal and led toâ… ,â…¢,â…¤wave, the waves were well differentiated, with the normal lantency. auditory neuropathy group was significantly different compared to other groups in the same condition. the data of auditory neuropathy group and normal control group of 20 ears otoacoustic emission results were analyzed by partitions of T-test,. Results show that the auditory neuropathy group and the normal control group otoacoustic emission amplitude of each frequency was no significant difference (P>0.05).the data of the wave lantencies of the general low-tone sensorineural hearing loss group, high-frequency sensorineural hearing loss group, normal hearing control group, were analyzed by partitions of T-test, Results show that the general low-tone sensorineural hearing loss group, high-frequency sensorineural hearing loss group and the normal control group latencies of each wave was no significant difference (P>0.05).Conclusions:(1)In auditory neuropathy cases, the hearing loss were mainly base on low-frequency, of moderate-severe hearing loss(2)With age growth, developing of disease, pure tone audiometry curves of auditory neuropathy showed decreased mainly full-frequency (flat type), are severe sensorineural hearing loss(3)The group of auditory neuropathy Tympanometry were A-type tympanogram; Stapedial acoustic reflex did not elicit.(4)TEOAE examination:in all cases have led to the frequency band, the band regeneration rate> 90%(5)ABR examination:one ear only leads to the extended latency of V wave, then none of ears leads toâ… ,â…¢,â…¤wave,(6)Cases of auditory neuropathy group 40HZ-related potential examination mostly lead to the normal waveform(only one ear not lead), the threshold increased.(7)Auditory neuropathy is an independent disease, his hearing characteristics are different from patients with sensorineural hearing loss.(8) The cause of auditory neuropathy, the pathogenesis may is different from the patients with general sensorineural hearing loss(9) Otoacoustic emission (TEOAE, DPOAE), ABR examination in the diagnosis of auditory neuropathy and differential diagnosis has the very important reference value..
