The Clinical Characteristics And The Surgical Treatment Of The Meningioma Among The Old

Objective:To discuss of the diagnosis,the surgery,the perioperative treatment and the treatment effect of the old's meningeoma through the patients over 70 in our section office from March 2002 to March 2010.Methods:To analysis the relationships among the surgery,the perioperative treatment and the treatment effect through the retrospective analysis of the clinical characteristics and the pathologic feature of the old patients with meningeoma from March 2002 to March 2010 in our hospital.Results:1. According to Simpson grade:36 cases of grade I resection,29 cases of grade II resection,9 cases of and over gradeâ…¢resection, ldead case after the surgery.2. After the surgery:10 cases with pulmonary infections,6 cases with arrhythmia,5 cases with digestive tract hemorrage,5 cases with new cerebral infarction,4 cases with intracranial hematoma,4 cases with DVT, 2 cases with cardiac insufficiency,1 cases with urinary infection,1 case with intracranial infection.3. Follow-up examination 3 months to 5 years:8 cases with neurodys-function,5 cases with hemiparesis,2 cases with anosmia,3 cases with visual field depletion.After the surgery:4 patients have recrudescence,1 cases with inside sphenoid wing meningeoma,1 case with slope meningeoma,1 case with meningeoma of fouth ventricle,1 case with meningeoma of parasaggital sinus.Conclusion:1. The incidence of the old's meningioma increased year by year. 2. The clinical symptom of the old's meningioma is atypical,most first symptoms are mild headache,dizziness or mental symptom and mild intelligence regression,lags in response.3. The most common histopathologic characteristics of the meningioma of the old is the fiber type,and the endothelial type comes next.4. There are many complications before the surgery, the hypertension and the diabetes mellitus are the most common ones.The incidence of the complications after the surgery is high including pulmonary infections,DVT,digestive tract hemorrhage,cerebral infarction, intracranial hematoma, urinary infection, hydrocephalus,and so on5. Because of the high brittleness of the old patients'intracranial vessels and brain tissue, avoid the overstretch during the surgery.For the tumor sites deep or in the life center,and the tumor suround the macrovessels, total removal of tumor is inadvisable.
