Objective:Tinnitus is one of the most common clinical symptoms of Otology, which refers to those who feel and hear sound in ears or heads subjectively but without sound source outside.17% of general population have this symptom in various degrees while 33% of old people may suffer from it. There are many kinds of illnesses which may cause tinnitus, and so does any illness in any part of auditory pathway. At present, the causes of quite a few patients are not only vague but also lack of objective diagnostic studies, which raises the curative difficulty. It is a pity that we can't release those patients'suffering. My thesis makes questionnaires on subjective tinnitus patients who accept examination of auditory function and about their conditions and depressive illness in order to know clinical features of tinnitus patients so that we can pave the way to cure the disease.Methods:Select 30 cases (39ears) of subjective tinnitus patients with ransom to make conventional examinations on pure tone audiometry,acoustic immittance measurement and DPOAE, and match frequency intensity of tinnitus, to make questionnaires on tinnitus and SDS. Select 30 cases (60ears) from healthy people with the same age and sex to make the same examinations. Then the two data will be compared and Statistic software package.Results:①Generally,30 cases aged from 13-69,14 women and 16men, primary education,secondary education,technical secondary school education and university education are 6.7%,70%,3.3% and 20% separately. Moods of 26.6% of the cases are affected by tinnitus while 30.8% of the cases have poor sleep quality.②Results of tinnitus questionnaire:66.7% cases suffer from monotone tinnitus while percentage of tinnitus of low frequency, medium frequency and high frequency are 25.6%,28.2%,and 46.25%. Subjective volume of tinnitus is divided into 6 levels. The majortiy of the cases belong to Level 5 who occupy 28.2%, level 1 and 6 take up 5.1%, the percentage of Level 2,3 and 4 is 23.1%,20.5% and 17.9%.③Results of tinnitus matching:the frequency matching of tinnitus shows that patients whose tinnitus frequency is 0.5KHz are the majority with the percentage of 35.9%(14/39),4kHz with the percentage of 28.2% (11/39).According to the result of the loudness matching of tinnitus, the average loudness of tinnitus is supraliminal 2.05 shi 1.64dBnHL, the largest volume is supraliminal 5dBnHL.④The morbidity rate of tinnitus patients who suffer from blues is 53.3%(16/30).The percentage of patients with mild blues is at most 40.0%(12/30),3 cases with moderate blues and 1 with severe blues. the incidence rate of tinnitus patients in the control group is 13.3%(4/30),and they suffer from moderate blues. When Chi-square test is made on incidence rate between tinnitus patients and the members of the control group, X2=13.017, p=0.001, you will find Significant difference between them.⑤DPOAE detection rate of 11 cases whose hearing is normal is 62.5%,while of the control group is 97.2%. Through Chi-square test the result is X2=20.280 p=0.000.which shows extraction rate of tinnitus patients' DPOAE is obviously lower than normal people and which indicates the morbidity of tinnitus may be related to the damage of outer hair cells.⑥The correlation analysis between the pain level of tinnitus (subjective loudness)and depressive situation indicates the rate of r is 0.245, p=0.132., greater than 0.05, which shows there is some what correlation but not too much. Because of short time and fewer samples further study will be done. The result of the analysis on the correlation between the pain level and objective loudness of tinnitus is that the rate of r is-0.002, P=0.989, which proves that there is no obvious correlation between them.Conclusion:1. The extraction rate of DPOAE of patients with normal hearing is obviously lower the control group, indicating tinnitus may be related to the damage of outer hair cells or its early performance. 2. The morbidity of tinnitus patients who are suffering from depression is clearly greater than the control group, which indicates the relations between depression and tinnitus are cause and effect.3. No correlation between the pain level of tinnitus (subjective loudness) and objective loudness suggests the subjective pain of tinnitus patients is not caused by tinnitus'objective loudness.4. There is some correlation between the pain level of tinnitus (subjective loudness) and depressive situation but not too much, which needs further study.
