Study On The Relationship Of Complement C₃,C₄ And Early Recurrent Spontaneous Abortion

Objective:To investigate the effects of complement and ACA in the pathogenesy of early recurrent spontaneous abortion, in order to well understand the immune pathogenesy of early recurrent spontaneous abortion, providing the new theory for the diagnosis and treatment of recurrent spontaneous abortion.Methods:1.Choose 30 cases who had the history of recurrent spontaneous abortion as research group,between them 15 cases who haven't pregnancy are research groupâ… ,15 cases who are having pregnancy but clinical diagnosis of inevitable abortion as research groupâ…¡;choose 30 cases normal women as contrast group,between them 15 cases who haven't pregnancy as contrast groupâ… ,15 cases who are having early normal pregnancy as contrast groupâ…¡.Research group and contrast group though ulnar vein get blood serum,using immunonephelometric assay to test complement C3,C4,using electrochemoluminescence immunoassay to test ACA.2.Using H & E staining and shortcut immunohistochemistry staining,to observe the histologic characteristics and complement C3,C4 expression in villus tissue of research groupâ…¡and contrast groupâ…¡.Results:1.The test result of blood serum sample:(1)The test result of ACA:research group ACA positive rate is higher than contrast group(P<0.05), research groupâ…¡ACA positive rate is higher than contrast groupII(P<0.05).(2)The test result of complement C3,C4:â‘ The complement C3,C4 of research groupâ…¡are higher than contrast groupâ…¡(P<0.05,P<0.05);The complement C3,C4 of research groupâ… ,contrast groupâ…¡,contrast groupâ… is no significant difference between those groups(P>0.05,P>0.05).â‘¡The complement C3,C4 are higher in the person of ACA positive than the person of ACA negative(P<0.05,P<0.05).(3)ACA is correlated positively with blood serum complement C3(R=0.616,P<0.05);ACA is correlated positively with blood serum complement C4(R=0.417,P<0.05)2.The test result of shortcut immunohistochemistry staining(1)The location of complement C3 expression in the villus tissue: About the villus tissue of research groupâ…¡,it is discovered that complement C3 is largely expressed at cell membrane and cytolymph of syneytio-trophoblast, langhans cell and mesenchymocyte; About the contrast groupâ…¡, it is discovered that complement C3 is little expressed at cell membrane and cytolymph of syneytiotrophoblast and langhans cell, dosen't express in mesenchymocyte.(2)The location of complement C4 expression in the villus tissue: About the villus tissue of research groupâ…¡, it is discovered that complement C4 is largely expressed at cytolymph of syneytiotrophoblast, langhans cell and mesenchymocyte; About the contrast groupâ…¡, it is discovered that complement C4 is little expressed at cytolymph of syneytio-trophoblast, dosen't express in langhans cell and mesenchymocyte.(3)The result of semiquantitative analysis:The complement C3,C4 express in villus tissue of research groupâ…¡are higher than contrast groupâ…¡(P<0.05, P<0.05).Conclusions:1.Patient of inevitable abortion blood serum complement C3, C4 are higher than the person of early normal pregnancy, existing complement over activation,it maybe is one of immune pathogenesis in recurrent spontaneous abortion.2.ACA is correlated positively with blood serum complement C3,C4, maybe complement activation is the first pathogenesy of ACA induced recurrent spontaneous abortion.3.Complement C3 and C4 are largely expressed at syneytiotrophoblast, langhanscell and mesenchymocyte in the villus tissue of inevitable abortion, which are different expressed in the villus tissue of normal pregnancy.4.Complement C3,C4 express in villus tissue of inevitable abortion are higher than normal pregnancy villus tissue,it demonstrated that patient of inevitable abortion had complement over activation.
