Objective:To investigate the clinical value of measuring plasma concentration of N-terminal pro-brain natriuretic peptide(BNP) in patients with acute coronary syndrome(ACS).Methods:A total of 112 patients diagnosed with ACS were included, and another 35 cases without heart diseases were included as the control group.(1) Patients of ACS were divided into ST segment elevation myocardial infarction (STEMI) group,Non-ST segment elevation myocardial infarction (NSTEMI) group and unstable angina(UA) group according to the examinations including echocardiography, biochemical indicators and clinical syndromes. (2) According to the criterion of Killip classification,patients of NSTEMI group and STEMI group were divided into Killip I group and Killip II group. (3) According to the examination of electrocardiography,patients of NSTEMI group and STEMI group were divided into LVEF< 50% group and LVEF≥50% group.The plasma NT-proBNP concentration was compared between NSTEMI,STEMI,UA and control group, between male and female in ACS patients,between LVEF< 50% and LVEF≥50% group,and between Killip I and KIllip II group. Peripheral vein blood of all patitens and volunteers were collected within 48 hours after admission. Plasma NT-proBNP,serum triglyceride (TG),total cholesterol (TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),apolipoprotein-a(Apo-a),and apolipoprotein-b (Apo-b) were measured with auto-biochemistry instrument.Results:1. Plasma NT-proBNP concentration in STEMI group,NSTEMI group and UA group was higher than those in control group(p< 0.05). The difference of plasm NT-proBNP concentration between STEMI group and NSTEMI group was not significant(p> 0.05). Plasma NT-proBNP level in NSTEMI group and STEMI group was higher than those in UA group(p<0.05).2. Plasma NT-proBNP consenctation obtained from patients with NSTEMI or STEMI was high gradually with the degree of Killip from I toⅡ(p<0.05).3. Plasma NT-proBNP concentration was positively related to age (r=0.275, p<0.05) and LVEDD (r=0.2, p<0.05),and negatively related to LVEF (r=-0.508, p<0.05). Measuring plasma NT-proBNP concentration is of great value in assessment of prognosis of ACS.Conclusions:1. Plasma NT-proBNP concentration was significantly elevated on the early stage of ACS. Plasma NT-proBNP levels is of great value in establishing the diagnosis of ACS.2. Plasma NT-proBNP concentration was positively related with Killip classification,LVEDD and age,and negatively related with LVEF. Plasma NT-proBNP concentration was high gradually with the severity of ACS. Plasma NT-proBNP concentration was useful in judging the severity of ACS. |
