Objective:To observe the efficacy and the changes of prostatic local micro-environment in chronic prostatitis(CP)treated with Transrectal Microwave Thermotherapy (TRMT) and analyze the correlation between the changes of factors and the symptoms.Methods:we were collected 160 cases of CP. The patients were randomly divided into three groups according the visit sequence:Drug group(55cases), the patients treated with antibiotic and a-blocker; Thermotherapy group(53cases),the patients treated with thermotherapy alone by the ZRL-II-A cavity intervention therapy instrument. The microwave thermotherapy was performed for 5days and the average temperature of TRMT is 42℃.Combination group(52cases), the patients treated with antibiotic, a-blocker and thermotherapy. The patients in each group were returned after 6 weeks.Before and after treatment,we observed the overall NIH-CPSI scores including the NIH-CPSI scores of the pain, the urinary symptoms, the quality of life and the categories. The level of Zn2+was detected by atomic spectroscopy method. Thiobarbituric acid (TBA) method was used to detect the malondialdehyde (MDA) and the contents of nitric oxide (NO) was detected by the nitrate reductase method. Superoxide dismutase (SOD) activity was measured using the xanthine oxidase method. Interleukin-8(IL-8),interleukin 10(IL-10),tumor necrosis factor (TNF-α),prostaglandin E2 (PGE2) andβ2-endorphin(β2-EP) was detected by the Elisa method.we'll analyse the relevance between various factors and symptoms.Results:According the criterion of efficacy judgments, the efficacy of the drug group, the thermotherapy group and the combination group were 54.5%,69.8%and 80.8% respectively. After treatment, the overall NIH-CPSI of three groups were significantly different than pre-treatment (P<0.01),between thermotherapy group and drug group has significant difference (P<0.01).The thermotherapy group and the combination group compared with the drug can significantly increase the concentration of Zn2+ (P<0.01).TRMT can also significantly decrease ROS levels and increase SOD contents.TRMT can also significantly reduce the PGE2 and increaseβ2-EP, especially suitable in the treatment ofⅢa andⅢb. There was a correlation between PGE2 and the symptoms of pain(r=0.650,p=0.011),while theβ-EP was negatively correlated (r=-0.747,p=0.000);TNF-a associated with urinary symptoms (r=0.619, p=0.002), while SOD was negatively correlated (r=-0.510, p=0.027);PGE2 and TNF-a correlated with improving quality of life (r=0.685,p=0.006; r=0.663,p=0.010), whileβ2-EP was negatively correlated (r=-0.595, p=0.040).Conclusions:TRMT through different ways changed the distribution and concentration of cytokines and enzymes, thus improving the local microenvironment of the CP. The correlation analysis found that the correlation existed between factors and the symptoms.
