ObjectiveTo observe the clinical curative effectof liver cirrhosis with cangniufangjihuangqi Recipe, compared with that of Western conventional treatment and that other prescription treatment. Discusses the mechanism of cangniufangjihuangqi Recipe on liver cirrhosis with ascites initially, to provide the beneficial reference for clinical treatment on liver cirrhosis with ascites reasonably.Method120 cases which come from the liver branch ward in guangdong TCM hospital and Guangzhou army hospital were randomly divided into three groups:the treatment group,the control group (1) and the control group (2), each including 40 cases.The treatment group were treated with routine western medicine adding cangniufangjihuangqi recipe, the cases in the control group (1) were treated by routine western medicine and other prescription, and the cases in the control group (2) were only treated by routine western medicine. If necessary, the therapy of drawing ascites with abdominal paracentisis could be adopted in order to reduce pressure of cavitas abdominalis in three groups. Two weeks were a treatment course.Recorded the main symptoms, the symptom (distension of the abdomen, fewurine, asthenia, accepts difference, jaundice, edema of the double lower limbs), the liver function, the body weight, the abdomen encirele, the abdomen B ultrasonic inspection (spleen thickness, Portal vein and spleen vein width) a time respectively before and after the treatment and the complication situation after the treatment, and monitored the 24h amount of urine, according to the corresponding standard of diagnosing and treating, maked the curative effect appraisal;observed the general medical examination for the body(for example T, P, R, BP), blood routine test, renal function and ECG untoward effect a time respeetively before and after the treatment, maked the secure appraisal. The comparison of general therapeutic effect:The total effective rate was 90% in the treatment group which was higher than 77.5% in the control group(2) and 87.5% in the control group (1); But after statistical analysis, the difference was not significant in three groups (P>0.05).2. The improvement of clinical symptom:the effective rate of the treatment group was unsignificantly superior to that in control group(1) in distension of the abdomen, asthenia, accepts difference, jaundice, but the effective rate of the edema of the double lower limbs was significantly; the effective rate of the treatment group was significantly superior to that in control group(2) in distension of the abdomen, asthenia, accepts difference, the edema of the double lower limbs;but there was not any significant in the symptom of jaundice; the effective rate of the controled group(1) was significantly superior to that in control group(2) in distension of the abdomen, asthenia, accepts difference, but the effective rate of the edema of the double lower limbs and jaundice, were unsignificant.3. Three groups of patients liver funetions, zymoplasm activity compairison:the effective rate of the treatment group was unsignificantly superior to that in control group (1) in ALT,AST,TB,pTA, but was significant in ALB; The improvement of liver function,zymoplasm activity compairison in treatment group and in control group(1) were both better than that in control group(2), which has significant difference between three groups according to statistic analysis (P<0.05). And the difference between the before and the after treatment were also apparent (P<0.05) in three groups respectively.4. Three groups of Patients body weights, circumference of abdomen the average 24h amount of urine comparison:Before and after treatment, the treatment group body weight, abdomen circles, the average 24h amount of urine change obviously, surpasses the control group(1) and control group(2), the statistic analysis has the significance difference (P<0.05);the comparison before and after the treatment the treatment group and control group (1) have the significant difference (P<0.05);the average 24h amount of urine comparison before and after treatment the Control group (2) has the significant difference (P<0.05), but body weight, abdomen circles don't change obviously, non-significant difference (P>0.05).5. The changes of supersonics at abdominal part:In the improvement level of spleen and vena portal diameter, the difference between the before and the after treatment were all also apparent (P<0.05) in three groups respectively.6. The complication after the treatment:After treatment the complication example belong to the three groups of patients, the treatment group and control group(1) are fewer obviously than the control group(2), have the significance difference (P<0.05). But there was non-significant difference in the treatment group and control group (1).7.Seeurity comparisons:After treatment three groups of Patients the general medieal examination for the body, blood routine test and kidney funetion, ECG, all not obviously change; in the course of treatment and aftertreatment, also no untoward effeet appears, indicating that the formula is safe and has no obvious poisonous side effeet for treating liver cirrhosis aseites.8.Rehospitalization after six months:Rehospitalization of 9 patients (include one cases of death) in treatment group was not significantly different to the rehospitalization of 16 patients (include one case of death) in the control group(2)(P>0.05). Nor to the control group(1), whose rehospitalization of 11 patients (include one case of death) (P>0.05). ConclusionClinical observation indicate that the treatment group using the eff formula to treat liver cirrhosis ascites show better iciency in curative effect improvement of symptoms resuming liver function than that in the control group and signs. And it also reduces the various complications, the recurrence rate, the pain and improves survival quality of life, in addition, it's safety and economy, is worthy to be popularized. The therapy of combination of Chinese traditional and western medicine has good effect on ascites due to cirrhosis.
