ObjectiveThe purpose of this study is to complete a retrospective analysis of COPD-inpatient databases in several Beijing hospitals from January 2000 to March 2010. It includes:(1) investigating the prevalence and characteristics of population distribution of CVD and its related diseases among COPD patients; (2) describing the changes in the development of CVD, gender differences, major cardiovascular diseases and their order; (3) discussing the prevalence of CVD among COPD patients and the tendency of the prevalence of CVD and relating diseases in the recent 10 years; and (4) examining the mortality risk factor of COPD, to provide evidence for better treatment of CVD among COPD inpatients.MethodsThe databases include information of hospitalized COPD patients in the following hospitals from January 1,2000 to March 20,2010:The General Hospital of the Liberation Army, Beijing Xiehe Hospital and Beijing Hospital.The databases provide general demographic statistics of the patients, major diagnosis ICD, medical expenses, etc. The data were cleaned using Excel and analyzed using SPSS 18.0.Results:(1) There were total 4960 COPD patients were investigated, of which 361 died in the past 10 years, accounted for 7.3%.(2) There are 2562 CVD patients.,48.8% of all COPD patients were diagnosed as a cardiovascular disease.The percentages of patients that are diagnosed to have 1,2,3 and 4 types of CVD were 27%,16.5%,5,6%and 2.6%, respectively.(3) The prevalences of male patients were 30.7% for ischemic heart disease, 23.7% for other types of CVD,10.1% for stroke, which were higher than those in female(all p<0.05). The prevalences of male patients were 22.5% for congestive heart failure,22.2% for chronic pulmonary heart disease and other disease of pulmonary circulation,46.8% for hypertension,20.0% for diabetes mellitus,8.7% for lipidemia, which were lower than those in female(all p<0.05). There were no significant differences in other comorbidity prevalences between two groups.(4) The prevalences of CVD and other related diseases has a tendency of increase as the age increase..For all age groups, hypertension has the highest prevalence among the CVD patients.(5)The prevalence of ischemic heart disease had a tendency of decrease while congestive heart failure and arrhythmia increase. The highest prevalence had a tendency to be found in less older patients except congestive heart failure. The prevalence of CVD were deceresed in male patients while increased in female patients.(6) All CVD were independent risk factors of mortality in COPD inpatients. The most common risk factors were ventricular flutter/ventricular fibrillation/ventricular tachycardia, chronic pulmonary heart disease and other disease of pulmonary circulation, stroke.Conclusion:CVD is common in elderly COPD inpatients, and the prevalence of CVD in COPD patients is higher than non-COPD patients.The prevalences of CVD were different between male and female COPD patients.The most common CVD were ischemic heart disease, other CVD, congestive heart failure in male while ischemic heart disease,congestive heart failure, chronic pulmonary heart disease and other disease of pulmonary circulation in female.The prevalence of ischemic heart disease had a tendency of decrease in COPD inpatients. But the percentage of female CVD patients were increased year by year.CVD is one of the risk factors of moatality in COPD inpatients.
