Abstract/Summary:
PurposeThe anterior communicating artery aneurysm is the most common aneurysm in the anterior circulation. The anatomical structure of this region is very complex, and that determines the surgery of anterior communicating artery aneurysm in the anterior circulation is still more difficult and high-risk. Therefore, preoperative imaging examination which can provide anatomical information of anterior communicating artery aneurysm is particularly important.Three-dimensional computed tomography angiography(3D-CTA), as a new diagnostic imaging technology in recent years, is given more and more attention by scholars at home and abroad in the neurosurgical field of diagnosis and treatment in cerebral aneurysms. This paper is to investigate the clinical value of 3D-CTA applied to the diagnosis and treatment of anterior communicating artery aneurysm.MethodThe imaging data of 85 patients who were confirmed by surgery with anterior communicating artery aneurysm in our hospital from February 2008 to September 2009 were analyzed retrospectively.26 patients were directly diagnosed by DSA, and confirmed by surgery with anterior communicating artery aneurysm.51 patients were directly diagnosed by 3D-CTA, and confirmed by surgery. The other 8 patients were examined with both 3D-CTA and DSA before operation, and confirmed by surgery.67 patients were examined with 3D-CTA after operation,and 9 patients were examined with DSA after operation.Results57 patients were first preoperatively examined with 3D-CTA.51 cases were diagnosed and confirmed by surgery. The other 6 cases were found to have an anterior communicating artery aneurysm by DSA not by 3D-CTA.28 patients were first preoperatively examined with DSA.26 cases were diagnosed and confirmed by surgery. The other 2 cases were found to have an anterior communicating artery aneurysm by3D-CTA not by DSA. Among the 85 cases, the detection rate of 3D-CTA is 89.5%, the detection rate of DSA is 92.9%, the detection rate had no significantly difference among the two methods (x 2-0.011. P>0.05)。85 patients were performed microsurgical treatment and all the aneurysms were clipped completely and assuredly. 73 cases passed pterion,12 cases through frontal lobe and longitudinal fissure.ConclusionThe 3D-CTA is a convenient, dependable and efficient imaging examination of the first choice for the diagnosis and treatment of anterior communicating artery aneurysm, not only for the choice of surgical approach to provide assistance, but also can be used as the routine examination after operation.
