Object:To evaluate and analysis the hardinge approach of total hip replacement.Methods:98 hips in 86 patients who have osteoarthropathy in the hip undergoing total hip replacement with the hardinge approach between July 2007 and July 2009 in our hospital were reviewed and analyzed . The period of follow - up from 1 month to 20 months, and the average period is 9.72 months . In - hospital data were collected retrospectively and the postoperative clinical situation were analyzed. At the same we use SPSS to analysis the correlativity between the degree of postoperative functional improvement and other factors for the patients who suffer from ischemic necrosis of femoral head and fracture of femoral neck.△Harris stand for the degree of postoperative functional improvement.Results:①86 patients(98 hips) have been followed up successfully. The average volume of perioperative blood transfusion(PBT) was 569.39ml. The average length of the incision was 12.98cm(7cm-15cm). The average period of postoperative walking functional exercise was 7 days(2 days -30 days). The average operative time was 168.27 minutes(80 minutes -350 minutes). The average period of in - hospital was 13.20 days(4 days -33 days). The average preoperative Harris score was55.65(poor)(29-74),and the postoperative was 87.57(good)(77-94). All the patients have neither nerve damage nor postoperative infection ,at the same time,they have neither postoperative dislocation nor phlebothrombosis of leg. The difference value of both lower extremities length is less than 1cm.②We have found that there is no relativity between the degree of postoperative functional improvement and other factors for the patients who suffer from ischemic necrosis. For the patients who suffer from fracture of femoral neck, there is no relativity between the degree of postoperative functional and operative time, period of in– hospital, period of postoperative walking functional exercise, PBT, gender. But the relationship between the degree of postoperative functional and the age is negative correlation , while the relationship between the degree of postoperative functional and length of the incision is positive correlation.Conclusion:①The hardinge approach is a kind of superordinary surgical approach with less injury and good exposure of the hip. It also has the superiority of less blood loss and short operative time. During the operation, operator has no need to division the extortor, allowing short period of postoperative walking functional exercise, which brings less postoperative complications and no requirements for special posture.②For the hardinge approach of total hip replacement in treatment for the patients who suffer from fracture of femoral neck, the older of the patients the worse the postoperative effect is.
