Objective:Compare the CSI monitored anesthesia in elderly patients with hypertension depth of anesthesia, and intraoperative hemodynamic changes and postoperative complications.Methods:Select 40 cases of general anesthesia in 65 years of age or older hypertensive patients, ASAⅡ~Ⅲgrade, age 65 to 78 years, the patients were randomly divided into four groups:①target-controlled infusion group (Tl group): the use of target-controlled TCI infusion of propofol induction and maintenance of rule of thumb control of the depth of anesthesia;②target-controlled infusion of the experimental group (T2 group): the use of TCI propofol target-controlled infusion induced and maintained, but the use of CSI under the control of the depth of anesthesia monitoring;③inhalation combined control group (I1 group): intravenous induction, mainly in the maintenance of sevoflurane inhalation, and the rule of thumb control of the depth of anesthesia;④inhalation combined experimental group (I2 group): intravenous induction, mainly in the maintenance of sevoflurane inhalation, CSI under the control of the use of depth of anesthesia monitoring. Observe and record the four patients during burglary (T0), after induction (T1), immediately after intubation (T2), after intubation 3 minutes (T3), skin incision (T4), surgery in the strongest stimulation (T5), Bi surgery (T6), extubation time (T7), the eyes open when you call (T8), fully awake (T9) of the heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), the depth of anesthesia indices (CSI), inhalation complex set of records minimum alveolar concentration (MAC) values, the last recorded count the total dosage, extubation time and recovery time, after 1 day of follow-up of post-operative complications. Statistical analysis using SPSS 13.0 statistical software for statistical analysis, measurement data to mean±standard deviation, said between the two groups, group t test was used to compare, count data usingχ2 testResult:Anesthesia during the four patients SBP, DBP, HR fluctuations are not large; target-controlled infusion group (T1, T2 group) and inhalation anesthesia group (I1, I2 group) compared to the dosage marked the high side, recovery time significantly partial length, the difference was statistically significant (P <0.05); the experimental group (T2, I2 group) and the control group (T1, I1 group) compared to the dosage marked the high side, and awakens in a long time, obviously biased, the difference was statistically significance (P <0.05); target-controlled infusion group (T1, T2 group), after extubation irritability, drowsiness, nausea, vomiting, number of cases is less than the inhalation anesthesia group (I1, I2 group), four patients had no intraoperative awarenessConclusion:Target-controlled infusion and inhalation anesthesia in elderly hypertensive patients may be to ensure the depth of anesthesia and hemodynamic stability, in which target-controlled infusion group of patients to maintain a large quantity of drugs, but the recovery time longer; inhalation anesthesia group surgery to maintain a small amount of medication, wake up faster, but the high incidence of postoperative complications.
