Clinical Analysis On Early-Stage Gastric Reoperation

Objective: to explore the reasons for the gastric reoperation and discuss the methods to improve the quality of gastric operation and lower the reoperation rate.Methods:survey the 1569 cases of gastric operation which were performed in Guangxi medical university first affiliated hospital during 1999 to 2009, Among which, 32 patients went through the gastric reoperation at the early stage due to postoperative complications. Through thorough analysis of these 32 cases, explore the reasons for the occurrence of gastric reoperation.Results:the early-stage gastric reoperation rate was 2.04%. 3 patients died. The mortality rate of gastric reoperation was 9.38%. The reoperation reasons are as follows. Intra-abdominal hemorrhage in 3 cases, intra-gastric stoma bleeding in 2 cases,postoperative ileus in 11 cases, digestive tract leakage in 12 cases, afferent loop obstruction combined with duodenal stump leakage in 1 case, intra-abdominal abscess in 4 cases.Conclusion:postoperative complications are the main reason for the early-stage gastric reoperation and the occurrence of postoperative complications is associated with improper handlings in the first operation, such as operational errors, incomplete stoppage of bleeding, unreasonable choice of operation types, etc. In order to lower the reoperation rate, operators should enhance their sense of responsibility, improve their professional skills in surgical operation and strictly follow requirements to perform operations. Besides, complete stoppage of bleeding and reasonable choice of operation types are the keys.
