The Clinical Value Of Colposcopically Directed Biopsy With Belinson Biopsy Forcep

Objectives:To investigate the the clinical value of colposcopically directed biopsy with Belinson biopsy forcep for cervical diseases and to compare the Belinson biopsy forcep with the traditional cervical biopsy forcep.Methods:A series of 399 cases who with cervicitis and cervical intraepithelial neoplasia (CIN) diagnosed by cervical colposcopic biopsy between June 2009 and March 2010. The 210 cases biopsy with the traditional cervical biopsy forcep,named traditional biopsy forcep group; the 189 cases biopsy with Belinson biopsy forcep,named Belinson biopsy forcep group. The operation time, the blood loss,the sizes of masses,the degree of pain and postoperative outcomes were observed.One month after biopsy bleeding, discharge, and repair were observed on the forth\seventh\fourteenth\twenty-first\twenty-eighth day.HR-HPV testing was realized by Hc2 before biopsy in 184 cases who were cervical intraepithelial neoplasia (CIN) and cervical cancer. The consistence of pathologic diagnosis between colposcopically dicreted biopsy and LEEP/CKC/hysterectomy/radical hysterectomy was evaluate between two groups. To evaluate the accuracy of colposcopically directed biopsy in the diagnosis of cervical intraepithelial neoplasia(CIN) cervical cancer.Results:The traditional biopsy forcep:the mean operation time was 1±0.3min (0.7-1.3min),45 cases (21.43%) felt painless,153 cases felt mild pain and 12cases felt moderate pain, mean size of biopsy masses was 32±9mm3, the average blood loss was 21±8ml(13-29ml),mean recovery time of the cervix 7.5±0.8(d)and virtually painless for most patients.The pathological dignosis after biopsy showed:109 cases inflammation,45 CIN1,26 CIN 2,30 CIN3,0 cervical cancer. 63cases underwent LEEP/CKC/hysterectomy/radical hysterectomy.The preoperative pathological diagnosis of 33 cases(52.38%)were consistent with that of the postoperative ones.The postoperative grade was lowered in 16 (25.40%) and was elevated in 14(22.22%). Belinson biopsy forcep group:the mean operation time was 0.6±0.2min (0.4-0.8min),152 cases (80.42%) felt painless and 37 cases felt mild pain, mean size of biopsy masses was 5±1mm3, the average blood loss was 9+ 4.5ml(4.5-13.5ml), mean recovery time of the cervix 4±0.8 (d)and virtually painless for most patients.The pathological dignosis after biopsy showed:116 inflammation, 37CIN1,17 CIN2,15 CIN3,4 cervical cancer and 38 cases underwent LEEP/CKC/ hysterectomy/radical hysterectomy. The preoperative pathological dianosis of 23 cases (60.53%)were consistent with that of the postoperative ones.The postoperative grade was lowered in 10 cases(26.31%)and was elevated in 5 cases (13.16%).Conclusions:1. The colposcopically directed biopsy has important value in diagnosing cervical diseases.2. The accuracy of diagnosing cervical diseases with colposcopically Belinson biopsy forcep directed biopsy is similar to the traditional biopsy forcep,it can be used as the traditional biopsy instrument.3. The Belinson biopsy mass is smaller, virtually painless for most patients, the operating, bleeding and recovery time of the cervix are also shorter (P<0.01).
