Background Diabetes is a metabolic disorder characterized by hign levels of blood sugar,it can influence the bones,nerves,kidneys and many other organs and tissues.The fractures with diabetes is very common in the clinic,the delayed union or nonunion rate and hign rate of pseudoarthrosis is a thorny problem faced by clinicians.As the development of cell biology and molecular biology,we pay close attention to bone morphogenetic protein-7 (BMP-7),transforming growth factor-β1(TGF-β1) and basic fibroblast growth factor(bFGF) during the fracture healing.However,we knew little about the change of BMP-7,TGF-β1 and bFGF in the healing of fracture with diabetes.However,we have not a further,unified understanding in the content and function of BMP-7,TGF-β1,bFGF during the process of fracture healing with diabetes.Objective To observe the change of BMP-7,TGF-β1 and bFGF during the healing of rats'fracture with diabetes,approach the molecular biology mechanism and find new fundament for the therapy of delayed union of diabetic fracture.Methods Seventy male Wistar rats(230-270g) were divided randomly into two groups:One was experimental group and the other was the control one. Streptozotocin(STZ)(60mg/kg)was injected into the enterocoelia of experimental group to induce diabetes model(the level of blood sugar >11.2mmol/L).Normal saline was given instead to the control one.The left tibia of all the rats were broken with fret-saw,and then treated with reduction and external fixation.Drew off serum and evaluated the healing of fracture by X-ray at 1,2,3,4,6,8 week.Tissue of fracture region were treated with HE staining to observe the skeleton formation and immunohistochemical method to detect the BMP-7,TGF-β1 and bFGF.ELISA was used to detect the serum of BMP-7,TGF-β1 and bFGF.SPSS16.0 statistical package was used to deal with all the data.There was significant difference when P<0.05 and highly significant difference when P<0.01.Results (1) The experimental group had little bony callus by X-ray at 1,2,3,4,6,8 week compared with the control one.(2) The experimental group was more hysteretic in bone formation than that in control one at each time point.(3)Immunohistochemical staining displayed that the content of BMP-7 had significant difference in the two groups at 2 week,The gray scales were 176.96±4.79 in diabetes group and 139.22±6.40 in control group(P<0.01);The expression of TGF-β1 and bFGF had significant difference in the two groups at 3 week,The gray scales of TGF-β1 were 189.59±2.76 in diabetes group and 166.74±1.33 in control group(P<0.05) and that of bFGF were 161.20±4.57 in diabetes group and 121.59±6.23 in control group(P<0.01).(4)The two groups' serum content of BMP-2,TGF-β1 and bFGF had significant difference respectively at 2 week-(1.77±0.23)ng/ml in diabetes group and (3.46±0.24)ng/ml in control group(P<0.01), at 3 week-(12.05±1.56)ng/ml in diabetes group and (17.48±0.56)ng/ml in control group(P<0.05) and at 3 week-(2.53±0.22)ng/ml in diabetes group and (4.25±0.24)ng/ml in control group(P<0.01).Conclusion The decrease of BMP-7,TGF-β1 and bFGF both in the serum and in the fracture region was one of the reasons of bad bone healing or delayed union in rats'fracture with diabetes. There were some synergistic effects possibly between TGF-β1 and bFGF.
