Objective:To investigate the safety and efficacy of transurethral therapy of superficial bladder tumor using 100W green laser.Materials and Methods:A total of 77 cases of pathologically diagnosed in patients with superficial bladder tumor using the 100W green laser were reviewed. Among these patients,42 were with single tumor and 35 were with multiple tumors, altogether 114 tumors. The tumors were with a diameter of 0.3 cm to 3.0 cm. Most of the cases were pathologically diagnosed as superficial bladder tumor by cystoscopic biopsy before operation, whereas some of the biopsies were taken intraoperation in consideration of possible hemorrhage, which might be difficult to stop bleeding. However, they were all staged Ta~T1。With combined spinal-epidural anesthesia, the shunkang washing cystoscope (23F,30°) was put in transurethrally before using the 100W green laser, with a averaged power of 100W,30°side fire,Results:Of the 77 patients, all tumors were vaporized successfully while no case needed an extra open surgery. The operative time was 10min to 60min, averaged 25 min. The operation view was clear, and no transfusion was required because of the little blood loss during the operation. Complications like serious secondary hemorrhage after operation, obturator jerk or perforation of the bladder did not develop. Catheter-indwelling was needed for 1 to 3 days and no bladder washing was necessary. The recurrence and progression were analyzed according to the characteristic of the tumors. After operation, mitomycin C or epirubicin, and BCG was used for bladder drainage. All patients were followed up for 6 to 18 months, averaged 13 months, with 7 cases of ectopic recurrence and no in-field recurrence, recurrence rate 9.1%. Conclusion:The unique advantage of performing and operating of the green laser make it simple, safe, and less complication develop in the treatment of superficial bladder tumor, it may also be safe on frail elderly patients; low recurrence rate and exact effect make the green laser an effective way for superficial bladder tumor.
