Objective:To evaluate the infectious risk factors and laboratory studies for the clinical diagnosis of postoperative intervertebral discitis of lumbar disk herniation, and to contrast the therapeutic effects of different treatments of discitis. Methods: From June 2002 to December 2009, 20 cases whom were clarified as postoperative intervertebral discitis of lumbar disk herniation at the First Affiliated Hospital of Fujian Medical University were analyzed retrospectively, compared with 1776 cases of lumbar disk herniation with surgical treatment at the above hospital during the same period. We investigated the Study Group and the Control Group by comparing the constituent ratios of gender, age, diabetes, hypertension, latent infection, cancer and HBV carriers with each other. WBC, Proportion of neutrophils, ESR, CRP of different treatment periods were collated between the two groups. We make comparisons between surgery therapies and Conservative treatments for discitis cases by comparing their costs, duration of treatments, and therapeutic effects based on the JOA Standard.Results:1.Gender, age and hypertension didn't show statistical significance in the incidence of postoperative intervertebral discitis of lumbar disk herniation. Also, the constituent ratios of diabetes patients between the Study Group and the Control Group were indifferent, however, the constituent ratios of latent infection, cancer and HBV carriers were found statistical significances between the two groups.2.WBC didn't present any specific meanings in the comparisons between the Study Group and the Control Group, but ESR and CRP did.3.There wasn't significant difference of costs and therapeutic effects between the surgery and the conservative treatments, but when the durations of the treatments was mentioned, the surgical therapies was much effective than the conservative treatments.Conclusions:1. Gender, age and hypertension were not considered as the infectious risk factors of postoperative intervertebral discitis of lumbar disk herniation, on the contrast, the diabetes, latent infection, cancer and HBV carriers did. If the glycemia situations of the diabetic patients could be well controlde before the surgeries, the possibilities of discitis can be obviously reduced.2. Body temperature, WBC weren't much helpful for the early detection of postoperative lumbar discitis. However, the changes of ESR, CRP were found more sensitively and could be regarded as the important indicators of evaluations of the patients during the observation period.3. Surgical therapies can shorten the course of treatments.
