ObjectiveIntervention in treatment of elderly hypertensive population in rural areas, to observe the angiotensin-converting enzyme inhibitors (ACEI) drugs Maleate Enalapril, the occurrence of adverse reactions cough epidemiological observations and to investigate the risk factors of Maleate Enalapril related cough.MethodsScreening of eligible rural communities, 45 to 75-year-old middle-aged essential hypertensive patients and give oral Enalapril Maleate Capsules,10mg, once a day, after 3 weeks observation, the first three weekends in the medication Cough questionnaire survey conducted. We record the time and characteristics of cough occurred. Adverse reactions based on whether there is a dry cough. The survey were divided into non-cough and cough group. To observe the distribution of population characteristics of ACEI cough and compare demographic characteristics of two populations, such as height, weight, waist circumference, hip circumference, body mass index (BMI), liver and kidney function, fasting blood glucose, triglyceride, cholesterol ester, high-density lipoprotein, low-density lipoprotein,glomerular filtration rate (GFR), Methylenetetrahydrofolate reductase Polymorphism (MTHFR C677T). Related factors in the above hierarchical cluster analysis.ResultsIn all included in the study included a total of 2335 cases and 53 cases lost to missing. Final completion of the survey data on the object of study of 2282 cases, male 832 cases, 1450 cases of women. In the follow-up process of a total of 201 (8.69%) cases were judged to occur and enalapril-related adverse reactions cough. These men 49 (5.9%) cases, 152 women (10.5%) cases. From the baseline point of view, the crowd of men and women in age, blood pressure, body mass index, total cholesterol, fat, triglyceride, high-density lipoprotein, low-density lipoprotein, blood urea nitrogen, creatinine and glomerular filtration rate in a comparison of both statistical Significance. The average level of the indicators of women were slightly higher than the men outside the age, blood urea nitrogen and creatinine. The general population group and the non-cough cough groups in age, blood pressure, body mass index, blood glucose, blood lipids (in addition to high-density lipoprotein), blood urea nitrogen, creatinine, and other indicators of comparison, the two groups were no statistically significant difference. Male population group and the non-cough cough group except blood glucose, triglycerides, the age, blood pressure, body mass index, total cholesterol, fat, ground-density lipoprotein, blood urea nitrogen, creatinine and glomerular filtration rate of the average difference between the two groups .There was no statistically significant. Female population in the cough group and non-cough group except age, high-density lipoprotein, the other indicators there was no statistically significant difference between two groups. Cough group of three MTHFR C677T genotypes TT, CC, TC distribution of frequencies were 21.5%, 51.0%, 27.5%, non-cough group were 24.1%, 50.6%, 25.3%, the difference was no statistical significance. Logistic regression analysis showed that age (OR=1.37,95% CI :1.07-1 .75) and women (OR=2.02,95% CI: 1.12-3.63), associated with the occurrence of cough. Group and non-cough cough compared with age and gender differences, elderly women and the occurrence of cough with ACEI. With body mass index, smoking, drinking, drug use history, blood pressure, high blood sugar, blood lipid levels, renal function levels and MTHFR gene polymorphism has yet to contact. ConclusionIn addition to the occurrence of ACEI cough, gender and age on the foreign, and obesity, smoking, drinking history, blood pressure, blood glucose, lipid levels, kidney function and MTHFR polymorphism has nothing to do. Women relative to men in taking ACEI antihypertensive drug class after the incidence of cough than men, women and men are taught very young age medication in patients with a high incidence of cough.
