Study On Admixture Of Potato Juice And Dexamethasone's Preventing Effects About Phlebitis Caused By Fluorouracil

Object:To research the Admixture of Potato Juice and Dexamethasone's preventing effects about phlebitis caused by fluorouracil.Methods:Animal experiment:46 rabbits (same variety, ordinary rabbits) were divided into 4 groups randomly. Experimental group (AG) and control group 1 (BG) contained 18 rabbits, control group 2 (CG) and control group 3 (DG) contained 5 rabbits. Four Groups all accepts chemotherapy with fluorouracil for three days. The puncturatio partis of experimental group were smeared by potato juice plus dexamethasone regularly (during the period of undergoing chemotherapy, smear medicinal liquid at the time of 0,30,60,90,120,180,240,360,480 min after intravenous injection; and after the end of the chemotherapy, smear medicinal liquid at 8:00 am everyday, Q 120 min, until the time of shearing specimens), whereas control group 1 were not and control group 2 smeared dexamethasone and control group 3 smeared potato juice regularly.72 hours after the third intravenous injection, observe the appearances of vasculars and tissues, then put the rabbits to death, cut vasculars and tissues around the vasculars (length 3cm, width 1cm), and make pathological section, observed through microscope at six aspects (inflammatory cells infiltration in vessel wall, the damaging of vessel wall, fibrous tissue hyperplasia in vessel wall, thrombosis in vessel, hemorrhage around vessel, edema of tissues around vessel). Pathohistological examination was performed on rabbit auricular vein in order to detect expressions of cellular adhesion molecule-1 and vascular endothelial growth factor at 24 hours after the last infusion. Clinical observation:100 digestive tract cancer patients who accepted chemotherapy with fluorouracil were divided into 2 groups (EG and FG), and each group contained 50 patients. To Compare the incidence of phlebitis of two groups.Results:Animal experiment:Visual observation:there was significant difference about phlebitis incidence rate between AG and BG (p<0.0001). Microscope observation:inflammatory cells infiltration in vessel wall:AG vs BG, the difference was significant (p=0.001), BG vs CG, the difference was significant too (p=0.04); the damaging of vessel wall:AG vs BG, the difference was significant (p=0.002), AG vs CG, the difference was significant too (p=0.001), AG vs DG, the difference was significant (p=0.048); fibrous tissue hyperplasia in vessel wall: AG vs BG, the difference was significant (p=0.008), BG vs DG, the difference was significant too (p=0.006); thrombosis in vessel:AG vs BG, the difference was significant (p=0.001), AG vs CG, the difference was significant too (p=0.0001); hemorrhage around vessel:AG vs BG, the difference was significant (p=0.0001), AG vs DG, the difference was significant too (p=0.01); edema of tissues around vessel:AG vs BG, the difference was significant (p<0.0001), AG vs DG, the difference was significant too (p=0.003).Immunohistochemistry:The expressions of cellular adhesion molecule-1 and vascular endothelial growth factor were increased in both groups (AG and BG) to a certain degree; however, the expressions of cellular adhesion molecule-1 and vascular endothelial growth factor in the experimental group were significantly less than control group 1 (p<0.05).In clinical observation, after smearing potato juice plus dexamethasone, the phlebitis incidence was lower.Conclusion:During the period of undergoing chemotherapy, smear medicinal liquid at the time of 0,30,60,90,120,180,240,360,480 min after intravenous injection; and after the end of the chemotherapy, smear medicinal liquid at 8:00 am everyday, Q 120 min, until the time of shearing specimens. According to the method above, smearing potato juice plus dexamethasone regularly, we can decrease the incidence rate of phlebitis caused by fluorouracil. The effect of the liquid on prevention and treatment of phlebitis may relate to the increasing expressions of cellular adhesion molecule-1 and vascular endothelial growth factor during habiting vascular damage. In clinical observation, during the period of undergoing chemotherapy, smear medicinal liquid (potato juice plus dexamethasone) regularly, we can decrease the incidence rate of phlebitis caused by fluorouracil.
