Objective:In a variety kinds of the Urinary Diversion and Bladder Reconstruction, the small intestine is the most popular using tissue. After the anastomoses of the intestines section and the urinary tract in the operation, the intestines section remains its nerves and blood vessel, which means that it still possesses the function of mucous secreting. When the mucous concentrated into the mucous plug in Urine, the plug may obstruct the uretha or the suprapubic catheter. This will increase the risk of the early period complication after the operation such as urinary tract infection and UTI; reduce the life quality of the patients; and influence the surgery effect. Now, different methods are adopted by many scholars both home and abroad to reduce the incidence rate of the mucous plug. For example, the chemicals as dehydrated alcohol, urea, Silver Nitrate, and N-etylcysteine are adopted to pour into the bladder, for the purpose to restrain the mucus secreting, but the results cannot be considered as ideal On the background of establishing the rat bladder substitution model, this experiments adopts the ordinary preventing and treating chemotherapeutics mdeicine for the bladder neoplasms, the mitomycin-C, to pour it into the bladder substitution model, observing the affects on different doses to the mucus secreting.Methods:Chosing the testing use SD rats on the weight of 250±20g, to separate them into control group, high dose mitomycin group, low dose mitomycin group, dehydrated alcohol group, and the sham operated group. Then establishing the rat bladder substitution model. To pour into the preliminary experiment selected dose of mitomycin and dehydrated alcohol respectively at the 8d,11d and the 14d after the establishing of the model. The urine specimen should be kept. When the urine specimen was taken at the last time on the 17d, to execute the rats. Taking the Ileal Neobladder and the normal ileum specimen, processing the HE coloretur, the AB-PAS histochemical stain coloretur, and the mucin MUC-2 immunohistochemisty coloretur. Measuring the total protein and the density of the sialic acid in the mucus of the urine, to observe the variances of the mucus secretory and the ileum goblet cells.Results:1.MMC has is less harmful for the injury of the Ileal Neobladder mucous membrane.(1). The HE coloretur of the tissue slice shows that, compared to the control group, there are some extent of damage do exist to the Ileal Neobladder mucous membrane and the mucous membrane substratum in the HMMC group and the LMMC group. But such damage is obviously lighter than in the dehydrated alcohol group.(2). The score and the rank test for the ileum mucous membrane: compared to the dehydrated alcohol group, the injuries in the HMMC group and the LMMC group is lighter, there exists significant differences.2. the MMC can make the amount of the goblet cells in the Ileal Neobladder increase, meanwhile, reduce the proportion of the blue-stain goblet cells:The AB-PAS of the tissue slice shows that:compared to the control group, the goblet cells in the HMMC group and the LMMC group have been increased, significant differences exist; compared to the control group, the blue-stain goblet cells in the HMMC group and the LMMC group have been reduced, significant differences exist, too.3. MMC can reduce the mucin concentration in urine.(1). Compared to the control group, in the HMMC group, the mucin concentration of urine all reduced at the 11d,14d and the 17d, there are significant difference.(2). Compared to the control group, in the LMMC group, the mucin concentration of urine reduced at the 14d and the 17d, there are significant difference.(3). For the mucin concentration in urine, the HMMC group is lower than the LMMC group at the 11d and 17d after the surgery, there are significant difference.4. MMC can reduce the sialic acid concentration in urine.(1). Compared to the control group, in the HMMC group, the sialic acid concentration of urine all reduced at the 11d,14d and the 17d, there are significant difference.(2). Compared to the control group, in the LMMC group, the sialic acid concentration of urine reduced at the 11d,14d and the 17d, there are significant difference.(3). For the sialic acid concentration in urine, the HMMC group is lower than the LMMC group at the 11d and 17d after the surgery, there are significant difference.Conclusion:1.in the HMMC group and the LMMC group, the mucin concentration in urine can be obviously reduced without visible damage to the Ileal Neobladder mucous membrane. And the effect in the HMMC group is more strong, which shows some dose of correlation.2.in the HMMC group and the LMMC group, both appeard the goblet cell hyperplasia and hypertrophy phenomenon. But compared to the control group, the proportion of the maturate goblet cess is reduced obviously.3.There are no significant difference to the normal intestine tissues and the goblet cells irrigated by the Mitomycin-C into neobladder.
