Literature Appraisal And Analysis For The Treatment Of Apoplexy Aphasia By Acupuncture

Study background:The apoplexy aphasia is one of common sequelae of apoplexy. It is an acquired speech disorder caused by cerebrovascular disease with organic pathological change occurred in language areas of the brain. Due to the method and results of evidence-based medicine are drawing more and more attention of the international medical community, the relevant methods of evidence-based medicine and system analysis are used to appraise the literatures to find the deficiencies in research work and clinical trials. It can be used as the basis of these kinds of advanced scientific research and design in the future, in order to find more effective acupuncture technique to treat the apoplexy aphasia.Obejective:The purpose of this study is to use the methods of evidence-based medicine and system analysis to appraise the scientific research and design and clinical curative effect of acupuncture on the treatment of the apoplexy aphasia.Method:Survey the domestic literatures for the clinical curative effect of acupuncture on the treatment of the apoplexy aphasia. Literatures indexing information:The papers published in 1980-2010 in China Journals Full-text Database (CNKI WEB). The involvement standard:(1) There was mention of the clinical treatment of apoplexy aphasia in the paper. (2) A random grouping design was used to ensure a more equal distribution between groups and have comparability. (3) A suitable sample size was used for the statistical analysis in the study/research. According to the principle and methods of clinical epidemiology/DME, establish "Systematic Appraisal Form of Literatures for the Treatment of Apoplexy Aphasia by Acupuncture". The statistical analysis is conducted in accordance with random, equal distribution between groups, inclusive standard, contrasting, sample size, statistical method, standardization of factor, and judgment standard of curative effect. Calculate the percentage of various kinds of situations, appraise its quality, and carry on relevant curative effect analysis.Result:There are total 83 papers related to the treatment of apoplexy aphasia by acupuncture, wherein 22 papers involve random contrast testing. The random quality is low, and only one paper is totally random.22 papers have "comparative property" among the groups. In the diagnostic standard,6 papers (1,6,12,15,18,20) adopt Chinese medicine and Western medicine to diagnose the cerebrovascular apoplexy, which accounts for 27.27ï¼….22 papers (1,2,3,4,5,7,8,9,10,11,13,14,15,16,17,18, 19,20,21,22) adopt Western medicine, which accounts for 100ï¼….11 papers (8,11,13,14,15,17,18,19,20,21,22) adopt "Chinese Aphasia Inspection Method" as the curative index to diagnose aphasia, which accounts for 50ï¼….22 papers involve the standard clearly, which accounts for 100%. 10 papers exclude the standard clearly, which accounts for 45.45ï¼…. The sample is not determined rigorously, Most papers do not carry on the statistical check, most of the statistical methods are correct.22 papers have the judgment standard of curative effect, which accounts for 100ï¼….7 papers (3,4,6, 7,8,15,21) adopt the head needle and the body needle in the experiment groups, which accounts for 31.82ï¼…and takes the first place. 5 papers (10, 11,13,14,16) adopt the body needle and the tongue needle, which accounts for 22.73ï¼…and takes the second place. 3 papers (2,12,22) adopt the tongue needle, which accounts for 13.64ï¼…. 3 papers (1,9,19) adopt the body needle, which accounts for 13.64ï¼…. 1 paper (5) adopts the composite treatment with head needle, tongue needle and body needle, which accounts for 4.55ï¼…. No bad response is reported in the papers. The effective value OR=4.56 for the treatment of apoplexy aphasia by acupuncture.Discussion: As for the randomized controlled trials (RCT) used in clinical scientific research according to the principle and method of clinical epidemiology/DME, several problems still need to be improved, such as the randomized distribution estimation method, calculation method of sample size etc. It should strengthen the scientific research and design, take randomized sample, correct sample size, increase the standardization of classificatory diagnosis, diagnose curative effect to classify judgment index, and observe bad response etc., in order to improve the intensity of verification, the analysis and comparison of curative effect.Conclusion: There is certain curative effect by using literature appraisal and analysis for the treatment of apoplexy aphasia by acupuncture. If there are more high-quality researches for the treatment of apoplexy aphasia by acupuncture, it will promote the argumentation intensity and the comparability of curative result.
