ObjectiveTo analyze the related factors of pancreatic fistula(PF) after pancreaticoduodenectomy(PD), in order to acquire more recognition of the PF after PD.MethodsA hundred and seventy patients who underwent PD consecutively at Shengjing Hospital from January 2003 to September 2009 were reviewed retrospectively. The management and outcomes of patients with PF were also evaluated. The factors which may potentially affect the incidence of PF were analyzed with mulivarite logistic regression model.ResultsOf the 170 patients, the overall morbidity were 37.05%(63/170),and PF occurred in 35 patients20.59%(35/170).PF was account for 55.56%(35/63) in the overall morbidity. Of the 35patients with PF,34 had successful management with effective drainage. In the 6 patients who had infra-abdominal abscess,1 patients died of secondum infectious shock. The mortality of PF was 2.94%(1/34). Multiariate logistic regression analysis revealed that pancreatic duct size(<3mm),carcinoma which infiltrate adjacent organ,tissue and blood vessels were independent risk factors(P<0.05).ConclusionsPancreatic duct size(<3mm),carcinoma which infiltrate adjacent organ,tissue and blood vessels are the independent risk factors of occurrence of PF after PD. Author presume that patient who have these risk factors should be carefully treated in the operation in order to reduce the occurrence of PF after PD.
