Objective:Noncompaction of ventricular myocardium (NVM) is a rare congenital myocardium. As the lack of understanding of the disease, it's often misdiagnosed or missed diagnosis. There's no specific clinical manifestation of NVM, and the ECG shows a variety of different performance. In this article, we described a case of isolated left ventricular noncompaction myocardium with ST segment elevation aimed to raise the awareness of this disease and to apply research base for future studies on electrophysiology of NVM.Methods:We collected the history and undergo the echocardiography, ECG and coronary angiography to diagnosis NVM. Furthermore, we discussed the mechanism of electro-dysfunction of NVM.Results:The patient was misdiagnosed as acute myocardial infarction because of the ST segment elevation. Through coronary angiography and echocardiography examination, isolated noncompaction of left ventricular myocardium was established.Conclusion:1. The diagnosis of NVM relies on echocardiography and cardiac magnetic resonance examination, as the lack of clinic manifestation. As a result, raising the awareness of the clinicians is helpful to improve the diagnosis.2. ST segment elevation results from many reasons, such as coronary artery disease, myocardium and so on. The history, physical examination, laboratory test and imaging examination should be considered combinedly to get the accurate diagnosis.
