[Objective] To evaluate the operative complications of tension free vaginal tape (TVT) and suprapubic arch sling (SPARC) for the treatment of female stress urinary incontinence(SUI).[Materials and Method] A systematic review of tension free vaginal tape (TVT) and suprapubic arch sling (SPARC) for the treatment of female stress urinary incontinence(SUI) was performed by searching the data bases including PUBMED, ELSEVIER,China National Knowledge Infrastructure and CBM.Literature references were checked at the same time. Languages was restricted to Chinese and English. Data including the rate of the bladder perforation,the plvic haematoma/bleeding,the bladder/vaginal erosions,the storage LUTS,the voiding LUTS and the reoperation, which compared TVT and SPARC,using the RevMan 4.2,and recommended each strategy,selection criteria,data collection and analysis.A meta-analysis was made.[Results] There were 9 studies including 730 patients with 2 studies of an adequately sampled single RCT and 7 studies of the low-quality RCT or observational studies.4 studies were RCT and 5studies were CCT.None of the CCT described randomization or usage of double blinded method.The results of comparing TVT and SPARC was that there was no statistically significant between TVT and SPARC in the rate of the bladder perforation(RR=0.68,95%CI=[0.40,1.18], p=0.17); there was no statistically significant between TVT and SPARC in the rate of the plvic haematoma/bleeding (RR=0.95,95%CI=[0.46,1.99],p=0.90);there was no statistically significant between TVT and SPARC in the rate of the bladder/vaginal erosions (RR=0.98,95%CI=[0.47,2.04], p=0.95);there was no statistically significant between TVT and SPARC in the rate of the storage LUTS (RR=1.27, 95%CI=[0.82,1.96],p=0.29);there was statistically significant between TVT and SPARC in the rate of the voiding LUTS (RR=0.71,95%CI=[0.52,0.97], p=0.03); there was no statistically significant between TVT and SPARC in the rate of the reoperation(RR=0.41,95%CI=[0.02,9.14], p=0.06).[Conclusion] The contrast of the rate of the complication between TVT and SPARC was that there was no statistically significant in the rate of the bladder perforation,the plvic haematoma/bleeding,the bladder/vaginal erosions,the storage LUTS and the reoperation,but the voiding LUTS.The rate of the complication between TVT and SPARC were the same in perioperative and postoperative.They had reliable efficacy and fewer complications of the treament of female stress urinary incontinence and minimally invasive surgical.
