Objective:To survey the content of serum total cholesterol(TC), low-density lipoprotein(LDL) and high-density lipoprotein(HDL) in patients with ulcerative gastric cancer. To study the difference of the level of serum total cholesterol, low-density lipoprotein and high-density lipoprotein between the patients with gastric ulcer, healthy adults and the patients with gastric cancer. To study correlation between the content of the three substance and the stage of gastric cancer. To study the correlation between the serum total cholesterol and the low-density lipoprotein, high-density lipoprotein in patients with gastric cancer. To study the changes of the three indexes before and after the operations in patients with gastric cancer.To analyze the reason and to provide reference for clinical therapy and prognosis.Methods:43 grown patients with ulcerative gastric cancer confirmed by gastroscope, biopsy and pathology were selected as the gastric caner group; 11 patients with gastric ulcer who were confirmed by gastroscope, biopsy and pathology and underwent operation were selected as the gastric ulcer control group; 20 healthy adults were selected as health control group. To reduce the interference of the surgical procedure,the patients with gastric cancer underwent D2 radical operation and BillrothⅡreconstruction, while the patients with gastric ulcer underwent subtotal gastrectomy and BillrothⅡreconstruction as control.Intraoperative frozen section examination was applied in patients in gastric ulcer group and postoperative routine pathological examination was applied in patients in both groups.One patient in gastric ulcer group was described as malignant tumor by frozen section examination and underwent D2 radical operation.He was confirmed as gastric cancer by postoperative routine pathological examination and was reassigned to gastric cancer group.2ml venous blood of every member was collected on an empty stomach. The blood of the patients with gastric cancer or gastric ulcer was collected 1 day before and 1 week after the operation. Then survey the contents of serum total cholesterol in enzymic method, and survey the contents of the serum low-density lipoprotein in direct method and high-density lipoprotein in PEG modifying enzyme method. The statistical analysis was made.Results:(1) Comparing with the gastric ulcer control group and the health control group, the contents of serum total cholesterol, low-density lipoprotein and high-density lipoprotein in patients with gastric cancer were significantly lower before the operation with a significant statistical difference(P<0.05), while the three indexes between the gastric ulcer control group and the health control group have no statistical difference(P> 0.05); (2) The contents of TC in patients with stageⅡ,Ⅲ,Ⅳgastric cancer were all lower than the gastric ulcer control group and the health control group(P<0.05); The contents of TC in patients among stageⅡⅢⅣgastric cancer had a significant statistical difference(P<0.05), and the contents of TC increased going with the decrease of the stage. The contents of LDL in patients with stageⅡⅢⅣgastric cancer were all lower than the control groups(P<0.05); The contents of LDL between the stageⅢandⅣhad no statistical difference(P>0.05), but were both lower than stageⅡ(P<0.05);The contents of HDL in patients with stageⅣwere lower than two control groups(P<0.05), but the contents of HDL between the stageⅡandⅢhad no statistical difference(P> 0.05).(3)The contents of serum total cholesterol and low-density lipoprotein, high-density lipoprotein had positive correlation(r=0.871, P<0.05; r=0.741, P<0.05).(4)The three indexes all increased after operation in the patients with gastric cancer with a significant statistical difference(P<0.05),comparing with themselves before operation, while the difference before and after operation in gastric ulcer control group was not significant(P> 0.05);Conclusion:(1)Comparing with the gastric ulcer control group and the health control group, the contents of serum total cholesterol, low-density lipoprotein and high-density lipoprotein in patients with gastric cancer were significantly lower before the operation, while the three indexes between the gastric ulcer group and the healthy adults group have no statistical difference.(2) The contents of TC increased going with the decrease of the stage of gastric cancer, and were lower than the two control groups. The contents of LDL with stageⅢandⅢwere lower than stageⅡ,but they were all lower than the control groups; The contents of HDL with stageⅣwere lower than two control groups, but the contents of HDL among stageⅡ,Ⅲand the control groups had no statistical difference.(3)The contents of serum total cholesterol and low-density lipoprotein, high-density lipoprotein had positive correlation. (4) The three indexes all increased after operation in the patients with gastric cancer, and the changes can provide the reference for the clinical therapy and prognosis.
