Objectives: A sub-project of New Village New family in the Grand Shangri-la Area of China named "one to one" maternal and child care action of Xiangcheng County was initiated by the National Population and Familing Planning Committee (NPFPC) at the end of 2008. The purpose of this study is to understand the status of maternal health service, health service use and the project implementation process, to indentify the problems or challenges and also to propose some feasible recommendations for future implementation.Methods: Both quantitative and qualitative researches were adopted in this research. The data from the quantitative study was obtained from process evaluation questionnaire survey of Grand Shangri-la project, to analyze the service provide capability and service utilization of 2007 and 2009.We both conducted in-depth interviews, focus group discussion and observed some health service facilities to get quantitative materials.Results: After one year's implementation, the "one to one" maternal and child care actions carried out very well with the operating mechanisms, but there were still some problems in this process. According to the implementation of five strategies of Health promotion, the project has made a great impact on reproductive health of pilot site and maternal health service utilization, the maternal health knowledge of women has been improved and the prenatal check and hospital delivery service utilization was also increased.Recommendations: After one year's practice, "one to one" has been proved to promote hospital delivery and maternal health service utilization, the following recommendations were proposed: (1) Scale up "one to one"maternal and child care action in the county;(2)The government needs to provide fund and policy to support the futher scale up of this project;(3) The public health center in township play an important role in rural reproductive health delivery, so the government should further building and improving capabilities of health care providers;(4) Strengthen health education of people, try to carry out peer education among pregnant women to improve the reproductive health awareness; (5) In the condition of hospital delivery couldn't be accepted by all the women, try to cultivate qualified rural midwives, to assist pregnant women delivery at home with new ways.
