Ultrasound Diagnosis Of Small Breast Cancer

Objective To investigate the features by analyzing the sonograms of small breasts nodules(≤15mm).The points for differential diagnosis of breast cancer through Logistic regression analysis were select and risk rank was disposed to improve the accurate rate of differential diagnosis of small breast cancer by Ultrasound.Methods 166 small breast nodules(≤15mm) confirmed by pathology were collected and retrospectively analyzed,a Logistic model for predicting breast malignancy on the basis of ultrasonographic feature was obtained and the risk degrees were ranked through OR values.Results(1) The Logistic regression analysis demonstrated that 9 ultrasonic features were enrolled in the Logistic equation,Logist(P)=-3.798+0.923X₁+2.038X₃+1.382X₅+0.495X₈+0.621X₁₀+0.823X₁₁+2.011X₁₂ +2.217X₁₃+1.564X₁₅.The coefficients were as follows:Indistinct and discontinue of cooper ligament,super fascia and lobule of breast 2.217,angular margin 2.038, microcalcification 2.011,shape taller than wide 1.564,spicular sign 1.382,absence of lateral shadow 0.823,posterior attenuation 0.621,uneven echogenicity 0.495,irregular shape 0.923.(2) The risk ranks and OR values were as follow:Indistinct and discontinue of cooper ligament,super fascia and lobule of breast(9.182)＞angular margin(7.675)＞microcalcification(7.471)＞shape taller than wide(4.776)＞spicular sign(3.982)＞absence of lateral shadow(2.277)＞posterior attenuation(1.861)＞uneven echogenicity (1.640)＞irregular shape(1.097).(3) The accuracy rate of the Logistic model was 89.2%.Conclusion The Logistic model of sonographic features such as indistinct and discontinue of cooper ligament,super fascia and lobule of breast,angular margin, microcalcification,shape taller than wide,spicular sign,absence of lateral shadow, posterior attenuation,uneven echo,irregular shape is helpful to differentiate the benign small nodules from the malignant ones.
