Objective To explore the clinical and radiological features of primary polyangitis , especially microscopic polyangitis with the main symptoms related to lung. Methods Six cases of primary polyangitis with the main symptoms related to lung were analyzed retro spectively. Results The initial respiratory symptoms included cough, excessive sputum , fever, dyspnea, haemoptysis , oppressed feeling in chest and chest pain. Chest imaging showed that multiplelaminar opaque and inter stitial fibrosis were common in those cases. Other symptoms included hemat uria, albuminuria, anemia, hypertension , skin lesion , joint lesion , dysfunction of liver andkidney ,etc. Antineutrophil cytoplasmic antibody was positive in all cases. One underwent renalbiopsy. Conclusion The lung is frequently involved in primary polyangitis. The clinical manifestationsare unspecific. Careful attention on the mouth , ears , eyes , skin , chest imaging , antineut rophilcytoplasmic antibodies , renal function and even biopsy is important for making diagnosis. Biopsy is the gold standard of primary polyangitis, and ANCA assay can be an important way to diagnose.
