Objective:To investigate the structure of interdomal fat pad by anatomical study and using ultrasonography(USG).Method:1.The 6 adult cadavers(4 women and 2 men cadavers) underwent primary rhinoplasty.The cadavers were dissected in the Anatomy Department of Peking Union Medical College using an open rhinoplasty technique.After removal of the fat pad,the vertical,transverse,and longitudinal sizes of the IFP were measured.2.The patients underwent primary rhinoplasties in the Department of Plastic Surgery at Peking Union Medical College Hospital. Ultrasonography was performed for 18 patients before surgery.Tissues located in the interdomal space were visualized.The vertical, transverse,and longitudinal distances of the IFP were measured sonographically.Result:1.All the cadavers and patients had IFPs occupying the interdomal space.2.The fat pad in the interdomal space differed from the subcutaneous tissue in the nasal tip.3.The mean vertical distance was 2.33±0.92 mm(range,1.1mm-3.7mm)。The mean transverse distance of the cadavers IFPs was 2.85±0.65mm (range,1.6mm-3.4 mm)。And the mean longitudinal distance was 11.54±1.51 mm(range,9.0mm-13.6mm) in the cadavers.The sizes of the IFPs were measured by USG before the patients underwent surgery.The mean vertical distance was 1.73±0.38mm(range,1.1mm-2.5 mm).The transverse distance was 2.33±0.53(range,1.7mm-4.0mm),and the longitudinal distance was 11.63±1.64mm(range,8.8mm-14.1mm)Conclusion:1.The IFP is an important anatomic structure in the nasal tip.2.A clear understanding of the effects of IFPs has on the dynamics of the nasal tip is mandatory for a surgeon to select the most appropriate surgical technique for each patient。3.Accepting the existence of this structure as a part of normal nasal anatomy may provide a new viewpoint for rhinoplasty.
