Objectives:1.According to retrospective study of clinical and pathobiological data of fifty-two cases of hypopharyngeal carcinoma,to explore related influencing factor of cervical lymph node and lung metastasis in hypopharyngeal carcinoma.2.To detect the expression of transcription factor Ets-1 and phosphorylated-Signal Transducers and Activators of Transcription 3(P-STAT3) in hypopharyngeal primary carcinoma and metastatic cervical lymph node and to study the correlation with the metastasis of hypopharyngeal carcinoma.Methods:1.Fifty-two cases of hypopharyngeal carcinoma patients who were treated by surgical operation in otolaryngology of Shan Dong Province Hospital from 2004 to 2006 were retrospectively studied.Analyze the correlation between gender,age, neoplasm's primary site,T classification,clinical stage,pathological classification and the cervical lymph node metastasis,lung metastasis.Immunohistochemical staining was adopted to detect the expression of Ets-1 and P-STAT3 protein in 52 cases of radically resected specimens of hypopharyngeal carcinoma and matched metastatic lymph nodes.Analyze the correlation between the expression of Ets-1 and P-STAT3 protein and clinical data,explore the relation between the expression of Ets-1 and P-STAT3 protein and the metastasis of hypopharyngeal carcinoma, and study the role in hypopharyngeal therapy. 3.Statistical software SPSS13.0 was used for statistical treatment.Fisher exact probabilities test or chi-s(?)are test was used for interclass difference test. Spearman rank correlation analysis was used for correlation test.Results:1.In 52 cases of hypopharyngeal carcinoma,the incidence of the cervical lymph node metastasis was 69.23%(36/52).The incidence of cervical lymph node metastasis in T3~T4 classification was significantly higher than that in T1~T2 classification(P<0.05),but there was no difference in gender,age,neoplasm's primary site,clinical stage and pathological classification(P>0.05).In 52 cases of hypopharyngeal carcinoma,the incidence of the lung metastasis was 15.38% (8/52).The incidence of lung metastasis in metastatic lymph nodes was significantly higher than that in nonmetastatic lymph nodes(P<0.05),but there was no difference in gender,age,neoplasm's primary site,clinical stage,T classification and pathological classification(P>0.05).2.The rate of protein expression of Ets-1 was 61.54%(32/52),which was significantly higher than that in normal hypopharyngeal mucosa which was negative(P<0.01).The rate of protein expression of P-STAT3 was 75.00%(39/52), which was significantly higher than that in normal hypopharyngeal mucosa which was negative(P<0.01).In 52 cases of hypopharyngeal carcinoma,the positive rate of Ets-1 and P-STAT3 in T3~T4 classification were significantly higher than that in T1~T2 classification(P<0.05,P<0.01);The positive rate of Ets-1 and P-STAT3 inⅢ~Ⅳstage were significantly higher than that inⅠ~Ⅱstage(P<0.05,P<0.01);The positive rate of Ets-1 and P-STAT3 in metastatic lymph nodes were significantly higher than that in nonmetastatic lymph nodes (P<0.05);The positive rate of Ets-1 in metastatic lung was significantly higher than that in nonmetastatic lung(P<0.05).In 36 hypopharyngeal carcinoma with metastatic lymph node,the rate of protein expression of Ets-1 and P-STAT3 in metastatic lymph nodes were 52.78%(19/52) and 58.33%(21/52) respectively; The positive rate of P-STAT3 in primary carcinoma was significantly higher than that in metastatic lymph nodes(P<0.01). 3.There was no significant correlation between the expression of Ets-1 and P-STAT3(P>0.05),but the co-expression of Ets-1 and P-STAT3 may increase the positive predictive value(92.31%) and negative predictive value(87.50%) of cervical lymph node metastasis in hypopharyngeal carcinoma.Conclusions:1.The incidence of cervical lymph node metastasis in T3~T4 classification was significantly higher than that in T1~T2 classification,the higher T classification may facilitate the lymph node metastasis.The incidence of lung metastasis in metastatic lymph nodes was significantly higher than that in nonmetastatic lymph nodes,cervical lymph node metastasis may facilitate the further metastasis after operation of hypopharyngeal carcinoma.2.The rate of protein expression of Ets-1 and P-STAT3 was significantly higher than that in normal hypopharyngeal mucosa,Ets-1 and P-STAT3 may play a role in the formation of hypopharyngeal carcinoma.3.The positive rate of Ets-1 and P-STAT3 in T3~T4 classification were significantly higher than that in T1~T2 classification;The positive rate of Ets-1 and P-STAT3 inⅢ~Ⅳstage were significantly higher than that inⅠ~Ⅱstage;The positive rate of Ets-1 and P-STAT3 in metastatic lymph nodes were significantly higher than that in nonmetastatic lymph nodes,Ets-1 and P-STAT3 may promote progress and cervical lymph node metastasis of hypopharyngeal carcinoma.The positive rate of Ets-1 in metastatic lung was significantly higher than that in nonmetastatic lung,Ets-1 may play a role in further metastasis.4.The co-expression of Ets-1 and P-STAT3 may increase the predictability of cervical lymph node metastasis in hypopharyngeal carcinoma,Ets-1 and P-STAT3 may be used as a clinical marker to predict metastasis potency of hypopharyngeal carcinoma and new gene therapy target of hypopharyngeal carcinoma.
