Tuberculosis (TB) is a chronic contagious disease which seriously damages population health. China is one of the twenty two countries with high disease burden caused by TB. The amount of TB cases in China is the top two in the world. The key of controlling TB is the early detection and cure. The epidemiologic sampling survey in 2000 showed that the prevalence is 367/100,000 for active tuberculosis, and 122/100,000 for smear-positive tuberculosis. The mortality of tuberculosis is 9.8/100,000. The infection rate of it is 44.5%. Tuberculosis has not been controlled effectively in China. It is far more complicated than just a public health problem.The prevalence of tuberculosis holds up the social development. It has been a serious disease which causes disease impoverishment, and slows down the economic development of rural areas. Population mobility is an important issue of controlling tuberculosis in the 21st century. The higher prevalence and severer morbidity of tuberculosis among the floating population are impeding the tuberculosis control.Based on the difference in social-economic development level, geographical difference and the amount of floating population we selected Qingdao, Dongying, Binzhou, Yantai, Weifang, Linyi, and Jinan in Shandong Province as study sample areas. Then from each area we chose twelve counties (cities or blocks) which were not included in the global fund to floating population program as the study sites. In each sample county, 2530 floating patients in tuberculosis were surveyed. The data were coded and managed by the researchers in Department of Social Medical Health Administrations Shandong University. They input, checked and analyzed the data with SPSS 15.0.In this study we surveyed the following characteristics: (1) demographic characteristics (age, gender, marital status and economic source and so on); (2) their knowledge and attitude on tuberculosis and related policies; (3) morbidity and treatment.Descriptive analyses were first conducted on the social demographic characteristics, knowledge on tuberculosis of patients. Then chi-square tests were applied to study on the factors which may influence the delaying diagnosis. Lastly, Logistic regression analysis was used to do the multi-variate analysis on the influencing factors.The main research results are as follow:1. The awareness rate of TB related knowledge is low. The effort in health education on prevention and treatment of tuberculosis is insufficient. The proportion of patients seeking for tuberculosis related knowledge is low.2. The misdiagnosis rate of the first visit to doctor among tuberculosis patients is high. Only 52.87% of tuberculosis patients were diagnosed in the first visit to doctor.3. The average delay of diagnosis among floating population is 18.84 days. For 40.76% of the patients, the delay of diagnosis is over 15 days. Patients attributed the delay in diagnosis to the following reasons: no realization of tuberculosis, being busy in work, no time, and financial problem.4. The reasons account for the hospitalize delay include amount of work days every week, health insurance, distance to the medical institution, being in a poverty-stricken family or not, and the initial morbidity. Strengthen the health education in prevention and treatment on tuberculosis to improve population incentive in prevention of tuberculosis and seeking medical services.2. Increase the coverage of medical insurance and medical aid, make the medical organization more systematical in order to increase to willingness of seeking doctor.3. Enhance the control on the prevalence of tuberculosis among floating population and provide services to protect them from being infected.
