The Evaluation On The Management And Quality Of Notifiable Infectious Disease Report Via Network In Beijing Medical Treatment Instituation

BackgroundIn accordance with the requirments of Department of Health, Beijing has reinforced the construction of disease precaution and disease control system in the past few years. In order to avoid misreport and improve the notifiable infectious disease report quality in hospitals, and to provide the effective reference to all of the various level leaders, we carry out this study.ObjectivesTo investigate the situation about notifiable infectious disease report management, evaluate the report quality, and find out problems emerged from daily work and increasing report quality.MethodologyThis research gathered information by interview, field questionnaire survey and collecting daily data. We interviewed the masters in charge of infectious disease report work in the investigated medical treatment organizations, carried out questionnaire survey on doctors and infectious disease reporters, collected cases from outpatient log, inpatient log and test registered in the lab of 2005 randomly, and found out the report card accordingly. The database was set up by Epi Data3.0 software and analyzed by SPSS 11.5 and Excel 2003.Main Results(1) we collected 5536 infectious disease cases randomly and surveyed 217 infectious disease reporters and 804 doctors when we investigated 52 medical treatment organizations. The result shows that the timeliness of the 52 medical treatment organizations is 94.18%, the consistency is 80.84%, the completeness is 88.47%, the misreport is 13.73%, and the integrating score of the report quality is 87.85. The grade two hospitals report quality is the highest which is the lowest in the grade three hospitals. Polyclinic report quality is higher than the infectious disease special hospital. The quality of the district is higher than the suburb. Inside medical treatment organization, the outpatient department report quality is the highest. The laboratory report quality is the lowest because of the high misreport as a result of server losing of the positive lab result.(2)The workload of the grade three hospitals is the most which is the lease in the grade one hospitals and township health centers. The management system, quality of personnel and report facility of grade two and three hospitals are better than the grade one hospitals and township health centers.(3) We used logistic regression to screen out factor that affect report quality. There came out four factors at last. They are servicing cards by doctor, receiving card by reporter, professional personnel training and rewards and punishment system.Conclusions and recommendations(1) The result suggests that the holistic report quality in Beijing is good. But there are differences between the grades and regions.(2) Presently, personnel training, rewards and punishment system and receiving card mode are the main factors that effect report quality.(3) The lack of laboratory report system is the chief reasons of misreport.(4) Strength the reporter troop is the trend in improving report quality.(5) Strength the informatization construction of infectious disease is the effective approach to improve report quality.
