OBJECTIVE To compare the effects of different transfusion volumes on balance of oxygen consumption during the cerebral surgery.METHODS ASAⅠ~Ⅱgrade neurosurgery patients with 40 cases, were randomly divided into two groups before operations(n = 20 cases),for the normal transfusion group(N) and the control transfusion group(C),and in accordance with groups to estimated amount of body weight per hour infusion. Before induction of anesthesia, intravenous infusion of compound sodium chloride injection, volume of the N group 10-12ml/kg/h,volume of the C group 3-5ml/kg/h.After induction, maintained the transfusion rate like before, ensured that the ratio of 1:2 plastic crystal, until the end of surgery.Monitored of HR,MAP,ECG,SpO2 at before(T0),after posting tube(T1), after tumor resection(T2),end(T3) time points, and after posting tube(T1),after tumor resection(T2),end(T3) time points, collected three jugular bulb blood to artery blood gas analysis and calculated so arteri-venous oxygen content difference(C(a-v)O2), cerebral oxygen extraction(COE),and lactate was measured.RESULTS The change of HR,MAP was calm in the C group. The lactate,C(a-v)O2 of C group was less than N group. The SvO2 of C group was more than N group. There is no difference from reduced COE of N group to C group at every time point.CONCLUSION 1.Control the volume of transfusion in cerebral surgery(3-5ml/kg/h),can maintain hemodynamic stability.2.Control the volume of transfusion in cerebral surgery(3-5ml/kg/h),can reduce the oxygen consumption of the promotion of brain oxygen balance, and to reduce the anaerobic metabolism.
