Objective To investigate the changes of brachium-ankle pulse wave velocity and related influential factors in patients with hypertension combined with obesity.Methods 5,146 patients conducting health examination in health management centre of the Third Xiangya Hospital from January 2007 to January 2008 were selected as the subjects.The patients were assigned to the hypertension combined with obesity group,hypertension group and obesity group with 300 cases in each group based on the matching of sex and age.The brachium-ankle pulse wave velocity(baPWV) was measured by VP-1000 arteriosclerosis diagnometer(Japan),and the blood pressure,body height,body weight,waist circumference,blood glucose, total cholesterol,triglyceride,high density lipoprotein cholesterols, creatinine and uric acid were all measured.All measurement data were presented as mean±SE((?)±s),the comparisons among the three groups were conducted by analysis of variance;meanwhile,the risk classification was conducted in the hypertension combined with obesity group to evaluate the changes of baPWV for different risks.For all patients,the Pearson correlation analysis and simple linear regression analysis were conducted to investigate the relationship between baPWV and systolic pressure,diastolic pressure,diastolic pressure,waist circumference and body mass index;and the multivariate stepwise linear regression of baPMV was conducted to take baPWV as dependent variable and the age, systolic pressure,diastolic pressure,pulse pressure,blood glucose, triglyceride,high density lipoprotein cholesterols,creatinine,uric acid, waist circumference and body mass index as independent variables. P<0.05 was considered as statistical significance.Results 1.Changes of baPWV of the three groups:The baPWV of the hypertension combined with obesity group(1635±239 cm/s) increased significantly compared with those of the hypertension group (1583±288cm/s) and obesity group(1453±119cm/s)(P<0.05);the baPWV of the hypertension group(1583±288 cm/s) increased significantly compared with that of the obesity group(1453±119cm/s) (P<0.01);and there were significant differences among these three groups (P<0.01).2.Changes of baPWV of the hypertension combined with obesity group with different risks:The baPWV of the high risk group and very high risk group(1705±259cm/s) increased significantly compared with that of the low risk group(1579±207 cm/s)(P<0.01).3.Single factor analysis between baPWV and blood pressure,waist circumference and body mass index:The Pearson correlation analysis was conducted between baPWV and systolic pressure,diastolic pressure, pulse pressure,waist circumference and body mass index,and the results indicated that the systolic pressure,diastolic pressure,pulse pressure, waist pressure and body mass index directly correlated with baPWV (systolic pressure r = 0.205,P value = 0.000;diastolic pressure r = 0.205, P value = 0.000;pulse pressure r = 0.524,P value = 0.000;waist circumference r = 0.175,P value = 0.000;body mass index r = 0.128,P value = 0.000).4.Analysis of correlative influential factors of baPWV:The multivariate analysis of baPWV was conducted to take baPWV as dependent variable and the age,systolic pressure,diastolic pressure,pulse pressure,blood glucose,triglyceride,high density lipoprotein cholesterols, creatinine,uric acid,waist circumference and body mass index as independent variables,and the results indicated that the systolic pressure, age,pulse pressure,waist circumference and body mass index are the main influential factors of baPWV(P<0.05).Conclusions 1.baPWV is one of the indexes to evaluate the changes of arterial elastic function in patients with hypertension combined with obesity.2.baPWV can be taken as one of the indexes to evaluate the risk classification of hypertension combined with obesity.3.baPWV is correlative with systolic pressure,age,pulse pressure, waist circumference and body mass index.
