Objective: Detect the expression of CK20 mRNA in the mesorectum of rectal cancer and study the relationship between the metastasis of mesorectum and the clinical pathology characteristic. Combination of hematoxylin and eosin stain ,explore the transfer rule of rectal cancer mesorectum and provide the reasonable excision basis for the security and radical operation.Methods: Cut 40 rectal cancer and its corresponding mesorectum tissues which were 1,2,3,4,5 centimeters from tumor tissues after rectal surgery.10 normal rectal mesorectum were also collected.Then:â‘ Detect the transfer of cancer cells in mesorectal by HE staining.â‘¡Through RT-PCR method, detect the expression of CK20 mRNA and analyze its expression according to clinical pathology datas.Results: In 40 cases of the mesorectum sections by HE staining, the positive rate of mesorectum cases of 1,2,3,4,5cm from the lower edge of tumor tissues were as follows: 37.5 % (15/40), 15.0% (6/40), 0% (0/40) and 0% (0/40), and 0% (0/40);The positive expression rate of CK20mRNA in mesorectum cases of 1,2,3,4,5cm from the lower edge of tumor tissues were as follows: 47.5% (19/40), 27.5% (11/40), 12.5% (5/40), 0% (0/40) and 0% (0/40). There were no CK20 mRNA expression in normal rectal mesorectal. The expression of CK20 mRNA in mesorectum were significantly related to tumor stages , tumor shape and differentiation.(P<0.05).Conclusion:â‘ The use of RT-PCR in detecting the expression of CK20mRNA (tumor micro-transfer) is more sensitive than the histopathology method;â‘¡For the TNM earlier staging differentiated adenocarcinoma tumor, 4cm is the relatively secure length of excision in the remote mesorectum.â‘¢Mesorectal cancer spread within the incidence increased with increasing TNM staging, and tumor morphology and tumor cell differentiation was positively correlated.
