Objective: To study the clinical guidance value of 64-slice computed tomography in patients who had underwent D2 lymph node dissection for advanced gastric carcinoma.Methods: CT data, the total of lymph nodes detected by CT preoperatively and lymph nodes diameter of diagnosed advanced gastric carcinoma patients were analyzed. The total of lymph nodes and lymph nodes metastasis rate of different diameter were compared with control group.Results: More lymph nodes were detected by 64-slice CT, especially for the lymph nodes which diameter is less then 10mm.The detection rate of lymph nodes d<10mm, d≤5mm,d≤3mm was 89.5%(111/124),65.3%(81/124),32.3%(40/124),respectively. Average excisional lymph nodes were 32 in CT group. The metastasis rate of lymph nodes with diameter d <10mm,d≤5mm,d≤3mm was 77.7%(108/139),49.0%(68/139),18.0%(25/139),respectively. Average excisional lymph nodes was 19 in control group, The metastasis rate of lymph nodes with diameter d < 10mm,d≤5mm,d≤3mm was 52.1%(25/48),27.1%(12/48),8.3%(4/48), respectively.Conclusions: 64-slice CT could detect more small lymph nodes preoperatively for advanced gastric cancer patients, which is an important reference for D2 lymph node dissection for advanced gastric carcinoma.
