Objective:To evaluate the efficacy of low tidal volume one-lung ventilation plus positive end-expiratory pressure.Methods:Thirty ASA classâ… orâ…¡patients were divided equally and randomly into two groups.During OLV,the two groups underwent two different ventilatory pattern of VT=10ml/kg,f=10f/min without PEEP (groupâ… )and VT=6ml/kg with PEEP of 5cmH2O(groupâ…¡).The airway pressure and hemodynamic parameters were monitored simultaneously.The arterial blood samples were collected for blood gases analysis immediately before one-lung ventilation,60min following two different OLV patterns and 24hours after the operation.Results:Ppeak was lower during low tidal volume one-lung ventilation plus positive end-expiratory pressure(P<0.01);and PaCO2 was much higher (P<0.01); PaO2 was higher(P<0.05);MAP,HR had no significant changes.Conclusions:During OLV, low tidal volume one-lung ventilation plus positive end-expiratory pressure can improve arterial oxygenation and decrease airway pressure.
