Objective:To assess the accuracy and the accordance of the left ventricular systolic movement in the patients with hypertrophic cardio- myopathy(HCM)by tissue synchronization imaging(TSI).Method:Sixteen patients who were diagnosed as HCM through analyzing and checking their disease history, clinical condition, echo- cardiograph, and electrocardiogram were carried out from July 2005 to March 2009 in our hospital. Among these patients, 11 non-obstructive HCM(6 male, 5 female), 5 obstructive ones(3male, 2 female), age 22-69 years, average age was 52.9±12.9 years. In addition, were carried out twenty healthy volunteers(11 male, 9 female)as controls, who were confirmed without diabetes mellitus and cardiovascular disease as well as the normal results of electrocardiogram and color echocardiographic diagnostic system, ages were from 47 to76 years, the average age was 58.8±10.4 years. In this study, we adopted the GE Vivid7r view ultrasonic diagnosis equipment to obtain the imagines of the apical four-chamber, apical two-chamber view and apical longitudinal views; Meanwhile, we collected 3-5 cardiac cycles under the normal heart beat and stored the images records in the disc, then analyzed the results without the equipment. At first, recording the normal results from echocardiography, and then transfer to the TSI modal. Septal, lateral, anterior, inferior, ante-septal and posterior walls were divided into basal and middle segments respectively in total 12 segments, the time to peak systolic velocity(TTP)of each segment was reflected by TSI mapping analying system through 3 different colors:green, yellow and red. And TTP of each segment was measured by quantitative TSI analyzing system. TSI measured TTP reflected that the time between the beginning of ventricular contraction and the peak time, which was equal to the time between aortic valve and open, and aorta valve closed after 200ms, and it was equal to the time of the rapid ejected (E-wave to start). The statistical analyses were conducted using SPSS16.0 software, data reflected material average + standard deviation(x_±s), t-test was not only used to analyze the differences between the two sample data materials, but also within each sample data, P value of <0.05 was considered statistically significant for all comparisons.Results: 1.Compaired with normal group, the clinical data of the HCM group showed that IVSd was 20.1±3.5mm, IVSd/LVPWd was 1.8±0.3mm, which were significally increase. LVDd, LVPWd, LVEF, peak velocities of E and A-waves, and E/A showed no difference. 2.The TTP of the bottom part and middle part of IVS in HCM group was 273.1±57.6ms, 281.2±69.1ms, compared with the normal group, the value of TTP delayed signifantly(p<0.01)in segment of septum and posterior wall(p<0.05). In the HCM group, the value of TTP delayed more signifanitly in the segment of septum than in posterior wall(p<0.05).Meanwhile, yellow color delayed could be seen in the wall ventricular septal of HCM patients on the TSI mapping analying system.Conclusions:HCM group have asynchrony of left ventricular move- ment, and TSI could exactly value it.
