Clinical Observation And Experimental Study On Compound Flumetason Ointment Treating Chronic Eczema

Bakeground and Objective: Eczama is a delayed allergy desease caused by exterior and interior factor. In acute phase, it is characterized by papulovescle, while in chronic phase, it is dominated by epidermis thickening, lichenification and pruritus. It is often divided into acute eczema, subacute eczema and chronic eczema clinically. Chronic eczema is often transformed from acute and subacute eczema, sometimes, it is caused by repeated scratching and stimulating. The course of chronic eczema is long and the people`s life is obviously affacted.There are many methods in treating chronic eczema, such as oral drug, topical applied, intravenous and intramuscular drugs. Topical applied glucocorticosteroid is the main method in treating chronic eczema. Compound Flumetasone Ointment is an external glucocorticosteroid compound preparation, which has anti-inflammatory and anti-anaphylaxis effect. In this study, clinical observation and mouse animal model experiment were performed to study the curative effect of Compound Flumetasone Ointment in treating chronic eczema and its mechanism.Methods:1. Clinical Observation76 patients with chronic eczema were from out-patient clinic, and Compound Flumetasone Ointment was applied to target skin lesion; Halometasone Cream was applied to the self symmetrical skin lesion as control. Compound Flumetasone Ointment and Halometasone Cream were applied to target skin lesion and the symmetrical skin lesion, twice a day. We observed curative effect on the 14th and 28th day after drug therapy. Therapeutic effect was assessed after 28 days.2. Experiment Study(1)Mouse model of chronic dermatitis induced by 2,4-dinitoflruorobenzene- (DNFB) In the experiment, 30 health BALB/c mice were selected, all mice were randomly divided into 3 groups of 10 mice each. 0.15% DNFB 25μL was applied to left dorsal ear of mice once a week and the course lasted 5 weeks. Base material, Compound Flumetasone Ointment and Halometasone Cream were applied to the left ear 30 minutes before and 3 hours after application of DNFB. At the first day, the left ear thickness was examined. Double ear thickness and weight were examined after experiment. Left ear samples were fixed in 10% formalin, paraffin imbedding, sliced and stained with haematoxylin and eosin or toluidin blue. Lymphocytes and mast cells were observed in dermis of the mice ears.(2)Mice model of chronic dermatitis induced by oxazoloneIn the experiment, 30 health BALB/c mice were selected, all mice were randomly divided into 3 groups of 10 mice each. Mice were sensitized by application of 100μL 1.5% oxazolone in ethanol to the abdomen. Then a total of 20μL of oxazolone dissolved in a mixture of acetone and olive(4:1) was applied to both sides of the left ear every 3 days after sensitization for 7 days, 3 times were over. Base material, Compound Flumetasone Ointment and Halometasone Cream were applied to both sides the left ear 30 minutes before and 3 hours after application of oxazolone. At the first day, left ear thickness was examined. Double ear thickness and weight were examined after experiment. Left ear samples were fixed in 10% formalin, paraffin imbedding, sliced and stained with haematoxylin and eosin or toluidin blue. Lymphocytes and mast cells were observed in dermis of the mice ears.Results:1. Clinical observation showed that the recovery rates of Compound Flumetasone Ointment group and Halometasone Cream group were 55.26% and 52.63%. The total effective rates were 85.53% and 81.58%. The comparison of curative effect between two groups was not statistically significant by x2-text(P>0.05).2. The result of mice experiment showed that Compound Flumetasone Ointment could down-regulate ear thickness and the difference between left and right ear weight after DNFB or oxazolone inducing chronic dermatitis of mice, and could reduce the numbers of lymphocytes and mast cells infiltrating in the dermis.Conclusion:1. Compound Flumetasone Ointment has good curative effect on treating chronic eczema and no obvious side effects.2. Compound Flumetasone Ointment could suppress inflammatory cell infiltration and topical inflammatory reaction in chronic dermatitis inducing by DNFB or oxazolone.3. Compound Flumetasone Ointment makes Flumetasone permeate into deep layer easily and increases drug action. Meanwhile, it has the effect of protecting skin barrier. Diphasic action make the curative effect notable of Compound Flumetasone Ointment.
