Relationship Between Low-grade Albuminuria And Subclinical Target Organ Damage In Nondiabetic, Essential Hypertensive Patients

Objective: To investigate the relationship between low-grade albumin- uria and left ventricular mass index (LVMI), geometry of left ventricle, carotid artery intima-media thickness(IMT), incidence of carotid plaque, glomerular filtration rate (GFR) in never treated essential hypertensive patients.Methods: 301 patients of EH were divided into three groups according to morning urine albumin concentration (MUAC):normoalbuminuria(NMAU) group (A group,n=107,MUAC<10mg/L); low-grade albuminuria (LGMAU) group (B group n=127,10mg/L≤MUAC<30mg/L); microalbuminuria (MAU) group (C group n=67, MUAC≥30mg/L). The body mass index, clinic blood pressure,body height and weight were measured for all patients.Fasting plasma glucose and plasma glucose 2 hour after oral glucose load, total cholesterol(TC), triglyceride(TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein-cholesterol(LDL-C), serum creatinine (Cre),urea nitrogen(BUN), uric acid(UA) were detected by automatic biochemica1 analyzer. Echocardiogram and carotid ultrasound examination were performed to detect interventricular septum thickness at end-diastole (IVST), LV end-diastolic posterior wall thickness(LVPWT), LV end- diastolic diameter(LVDd), IMT of carotid artery, incidence of carotid plaque and PI index of carotid artery.Results: Logistic multiple regression analysis showed that BMI (OR=1.098, P=0.037), systolic blood pressure(OR=1.100, P<0.000), FBG (OR=2.323,P<0.000), TC(OR=2.159, P=0.003), HDL-C(OR=0.539, P=0.014) were independently associated with the occurrence of low-grade albu- minuria. Compared with normoalbuminuria group, IVST, LVPWT, RWT, LVMI, incidence of concentric hypertrophy and concentric rem- odeling of left ventricle ,IMT and PI index of carotid artery and incidence of carotid plaque in low-grade albuminuria group were significantly increased (p<0.01). GFR was significantly decreased (p<0.01).Conclusion: low-grade albuminuria was positively related to sub- clinical target organ damage in essential hypertensive patients.
