Clinical Significance Of The Infiltration Of CD20-Positive Lymphocytes In Renal Allograft Pathology And The Distribution Of TCM Syndrome Of The Patiens

【Objective】(1) To analyze the infiltration of CD20-positive lymphocytes in the reject reaction of renal allograft,correlate the results with graft survival.(2) To explore the distribution of Traditional Chinese Medicine(TCM) syndrome of the patiens with the infiltration of CD20-positive lymphocytes.【Methods】We examined 93 integrated follow up data biopsy samples from 129 renal allograft patients biopsied in Department of Urology,Fuzhou General Hospital, Nanjing Military Area Command from February 2003 to December 2008.The male is 67 and female is 26.CD20 was assayed by immunochemistry staining.These cases were divided into three groups according to the degree of CD20 positive cell infiltration with semiquantitative method:severe group(I＞50%area),moderate infiltration(≥10%,＜50%area)and negative group(＜10%area).To analyze the infiltration of CD20-positive lymphocytes in the reject reaction of renal allograft, correlate the results with graft survival,to observe the infiltration of CD20-positive lymphocytes patients who treatmented by Traditional Chinese Medicine(TCM) and explore the role in renal allograft.Patient graft status was followed postbiopsy and documented for graft survival or failure using the cutoff date of December 31, 2005.All cases were to collect information of treatment,serum creatinine(SCr), albuminuria and graft survival.【Results】Three groups,severe group(I＞50%area n=20),moderate infiltration(≥10 %,＜50%area n=25)and negative group(＜10%area n=48) in all 93 patients. (1)Three groups have no difference with age,gender,biopsy time and times of transplantation(P＞0.05).(2) In the twelfth month of the following-up,the average SCr of severe group(466.50±330.53μmol/L) was higher than that ofmoderate group (360.16±290.30μmol/L) P＜0.05;the moderate group was higher than that of negative group(276.79±240.78μmol/L) P＜0.05.(3)The graft survival ratio of severe group was lower than that of moderate and negative group(P＜0.05) in 4 years.(4) The ratio of the biopsy patients with albuminuria of severe group was lower than that of moderate and negative group(P＜0.05) in 12 months.5) Syndrome research:The distribution proportion of TCM syndrome of the patiens with the infiltration of CD20-positive lymphocytes as fllows:Liver-kidney yin deficieney (33.33%),blood stasis syndrome(26.67%),Liver-kidney qi deficieney(17.78%), syndrome of qi-yin deficieney(13.33%),spleen-kidney yang deficieney(6.67%), syndrome of yin-yang deficieney(2.22%).【Conclusion】The degree of CD20 positive cell infiltration in the renal allograft was a important factor for allograft prognostic.The distribution proportion of TCM syndrome of the patiens with the infiltration of CD20-positive lymphocytes as fllows:Liver-kidney yin deficieney(33.33%),blood stasis syndrome(26.67%),Liver -kidney qi deficieney(17.78%),syndrome of qi-yin,deficieney(13.33%),spleen -kidney yang deficieney(6.67%),syndrome of yin-yang deficieney(2.22%).
