Objective To study the prevalence of adverse childhood experience and smoking and the influencing factors of the medical college students in Anhui Province ,and to inquire into the relation of smoking and the influencing factors, and to provide bases for the prevention measures ..Methods Using stratified cluster sampling method were chosen and investigated by an anonymous and questionnaire, 810 students self-administrated questionnaire. Chi-square analyze the correlation test, Z test, nonparametric test and logistic regression analysis were applied to prevalence of adverse childhood experience and smoking behavior and its correlation.Results The prevalence of adverse childhood experience was very universal high among these students. The prevalence of physical abuse was 31.3%,which is the highest. The prevalence of pamily member went to prison was 0.9%.Over half of respondents reported at least 1 of 10 categories before 18 years old. Over one third of respondents reported at least 2 of the 10 categories.The distributions of ACE were different among different sex and different borning place. The prevalence of sexual abuse and parental separation and divorce of female students were higher than that of male ones(P<0.05). The rate of physical and emotional neglect of male students were higher than those of female ones(P<0.05). The prevalences of physical abuse and parental separation and divorce students from city were higher than those from rural area and town(P<0.05), while the rate of physical neglect and family violence of students from rural area was higher than those from city and town(P<0.05).The different levels of father's and mother's education had different prevalences of ACE.There are 758 students investigated, and total prevalence rate of smoking behaviors was 26.4%,which was 51.2% for male and 2.58% for female . The age of beginning smoking was 18.62±2.965 years old . Smoking behavior was closely related to sex,the level of mother's education, smoking of father or mother and drinking.The protective factors to somking were female gender and higher level of mother's education,and the risk factors were smoking of father or mother, witness family violence and drinking .Smoking was related to the individual event of ACE. The risk factors were emotional abuse,sex abuse, emotional neglect ,family violence,family matter abuse, parental separation and divorce.With the increasing of the ACE score,the risk of smoking was also increased.Conclusion Smoking is popular among medical college students. Health promotion program should be carried out to publicize the hazard of smoking behavior and healthy living styles .
